Need help... What do I ask my RE? (*trigger warning)

Hi ladies,

I feel like I should apologize in advance for the long post.  Sorry, but thanks for your help!!!!

I have been a lurker on this page for quite a while.  I feel like I have been around the block and back again.  My husband and I feel like we have hit an infertility wall, and I need some guidance from you all.  You all seem to have such an array of experiences and I could really use some insight from you all.  I have an appointment with my RE next week and I need to put together a list of questions or general things to talk to her about.  

Here is my story, for background info on what has already been tried.

I got pregnant with my son in 2010 and he was born in 2011.  My pregnancy was pretty uneventful, my delivery was quite a different story.  I labored for 48 hours and in the end I had to have an emergency c-section.  After leaving the hospital, I developed post-partum pre-eclampsia.  I also developed an infection.  I was hospitalized for an additional 4 days.

Fast forward 2 years (2013), my husband and I try to conceive again.  We try for several years without any positive pregnancy tests.  We finally decide that we should seek additional help and talk to my gynecologist (2015).  My doctor schedules an HSG where they find what they think is scarring.  She refers me to a RE.

In September 2015 I meet with my RE. She diagnoses me with PCOS and she also schedules a hysteroscopy and a laparoscopy. During the surgery she doesn't find scarring but actually multiple polyps.  She removes the polyps and restores my uterus to its normal state.  

I move on with 3 rounds of timed intercourse with clomid.  All of these resulted in BFNs.  I then tried 4 rounds of IUI with clomid.  All of these resulted in BFNs.  (My husband's sperm was not an issue - he has at least 50,000 sperm for each of the IUIs.)

In the summer of 2016 we decide to go ahead with a round of IVF.  My RE was able to retrieve 46 eggs.  Of these eggs, I had 16 make it to day 5.  We decided to PGS these 16 eggs.  Of these, 8 were chromosomally normal.  All 8 of these eggs were frozen.

FET #1 August 2016 - We use estradiol only protocol.  My lining was grown to 7.9.  I used crinone progesterone.  We transfer 1 AB embryo.  This resulted in a BFN.

FET #2 September 2016 - We use an estradiol only protocol.  My RE changed up my progesterone to PIO (1ml).  My lining took a bit longer to grow, my transfer is pushed back a week until it reaches at least 8. I was taking 10 mg of estradiol. (2 morning, 2 night, 1 vaginally) I get it to 8.2.  We transfer 1 AB embryo.  Transfer results in a BFP.  Beta #1 86; Beta #2 (4 days later) 101; Beta #3 25.  Chemical pregnancy.

FET #3 November 2016 - We use an estadiol only protocol.  My RE continues with PIO (1ml).  My lining again took a bit longer to grow. She ups my estradiol to 10 mg of estradiol again. (2 morning, 2 night, 1 vaginally).  My lining got to 8.3. We transfer 1 AB embryo.  Transfer results in a BFN.

FET #4 December 2016 - We use an estradiol only protocol.  Again we continue with PIO (1 ml).  My lining again takes longer to grow and she raises my estradiol to 10 mg again.  (2 morning, 2 night and I vaginally)  My lining this time gets to a 10.  We decide to transfer 2 embryos - 1 AB and 1 BB.  Transfer results in a BFP.  Day before Beta #1 I was able to get a digital home pregnancy test to turn positive.  Beta #1 25; Beta #2 22. Chemical pregnancy.

We currently have 3 embryos remaining and we really want to try to do some kind of additional testing but we really don't know where to start.  Again, I have a 30 minute consultation with my RE next Thursday, but I really want to come to this meeting with some suggestions. 

So far, all I can think to ask about is Natural Killer Cells.  What else do I need to ask, bring up, suggest? What tests should I discuss with her?  What else should I think about?  I truly can't thank you ladies enough!

Additional info: I am 37 years old and weigh 195 pounds.

June 2015 - referred to RE - diagnosed with PCOS and suggests laparoscopy and hysteroscopy for possible scarring. Insurance doesn't cover so we must wait until new insurance kicks in. 
Sept. 2015 - Hysteroscopy and laparoscopy to remove many polyps and unfuse Fallopian tube from ovary. 
Oct. 2015 - TI w/ clomid - BFN
Nov. 2015 - TI w/ clomid - BFN
Dec. 2015 - TI w/ clomid - BFN
Jan. 2016 - IUI w/ clomid - BFN
Feb. 2016 - IUI w/ clomid - BFN
Mar. 2016 - IUI w/ clomid and ovidrel - BFN
Apr. 2016 - IUI w/clomid - BFN
May 2016  - decide to move on to IVF
June 2016 - start  BCP, Lupron and Follistim - meds work a little too well estradiol level over 6000 on day 8 of stims
July 5, 2016 - ER - retrieve 46 eggs, 42 are mature. 28 fertilize with ICSI, 26 on day 3, 16 on day 5 biopsied and frozen. PGS testing show that 8 are normal. 
July 2016 - start FET #1 - estradiol and progesterone
Aug. 8 - transfer 1 embryo - BFN. 
Sept. 15 - transfer 1 embryo - BFP beta #1 86, beta #2 101 mc 5w5d
October 28 - transfer 1 embryo - BFN
December 19 - transfer 2 embryo - BFP beta #1 25, beta #2 20, beta #3 120 beta kept climbing, but no baby mc 8w3d
Long needed break January - August (During the break we had ERA, recurrent loss blood work and saline sono.)  Hooray for receptivitiy!!!
Sept. 6 - transfer 1 embryo - BFN

  • If I were you I would 100% ask for an ERA test.  Have you heard about it?  Here's the link that explains it better than I can-

    A few of us had it done over the past few months.  There is a recent thread on here regarding it.  It's very new but my RE has seen it help quite a few of his patients who are having multiple failures when everything looks like it should results in a BFP.  Let me know if you have any questions about it!

  • This sounds like something that my nurse mentioned to me in passing but never with and suggestion to do it. Thank you so much for the link. I will definitely be trying to find out more information about this. 

    How costly was it? It can't be cheap.

  • You're welcome! I believe it was about $1800 all together?? You pay your clinic for the biopsy and then the testing place for the test.
