Any other T1s TTGP?

Just looking for some other ladies to relate to. This is DH and I's second month TTPG. I was a frequent around here about 6 years ago but am back with an account that doesn't have so much personal info associated with it.

I got the blessing from my endo to TTGP in August with an A1c of 6.9. Still working on bringing it down further, aiming for 6.0 (lowest ever was 6.1). I have a Tandem T:Slim with an integrated dexcom G4. We met with a MFM 6 years ago and he just retired so we're on the hunt for a new dr.

Show of hands, how many other T1s are out there? How long have you been TTGP? A1c goals? Anyone else on a pump and/or CGM?

  • I'm Type 1 for 22 years. I am now 11 weeks pregnant with my first. It took me 7 months to get pregnant. My A1C while TTC was 6.3 and now I have it down to 5.7. I'm on a pump (Animas) and a CGM (Dexcom G5).

  • @krueckemeier Nice work on the A1c!! I had an Animas for almost 15 years and was pretty happy with it. Congrats on the pregnancy, I hope all is going well :)

    Hi there, it's great to find other T1s on here! This is my first pregnancy (6 weeks). My A1C was 6.7 when I conceived, which is not great... Previous to that I had stayed around 6.3 for a couple years. We had been TTC for 5 months when it happened, and I was discouraged and not taking care of myself as well as I should have - of course, that's when it happened! I'm really nervous about pregnancy and keeping my BG in range and have been eating very low carb and getting great numbers. Luckily I have a Tslim and a Dexcom G5 - thank goodness for CGMs! Would love any tips or advice from you gals!
  • @SDWE I just responded to your post on the Aug 17 board! My last A1c was 6.8 so not the best but I truly believe it's not a true reflection of my current control (much lower numbers overall). 

    I'm currently 11 + 3 and the first few weeks after my BFP were highly emotional with concerns over every high number (even sobbing on the couch at 3am when my BG just wouldn't respond how I wanted). My biggest advice is to try the best you can but don't be too hard on yourself for past numbers.

    Also, carb needs increase during pregnancy (my dietician recommends 200-250 carbs per day for my calorie goals). And with pregnancy, if you're not consuming enough carbs, you can actual begin spilling ketones with even a normal BG. So be sure you're eating enough carbs and covering with insulin.

    And despite what they say about insulin needs and when they rise and fall, each person is still an individual. So if you're seeing trends, don't be afraid to tell your endo. Struggling with lows or highs and feeling like their your fault when really i:c ratios, correction ratios, or basal rates are just due for adjustment causes are lot of unnecessary stress aand frustration.

    And lastly, temp basal rates are your friend!! For me, a 10% increase for sticky highs or 10% decrease for those days where I just keep bottoming out are life saving (literally!).

  • Hi all! Due in Feb17 here. My review of T1D pregnancy so far:

    Weeks 1-24... required effort, but went awesome. It was so helpful to have a good Endo team and to use them :). My A1C stayed at 5.8. I was trying hard, but this was my opinion on my blood sugar:


    Weeks 24- now (32). Much, much more challenging for me, but YMMV.  Overall still going fairly well,but the effort required has gone up tremendously. I added a CGM; I had tried a Dexcom twice before with annoying site-pain reactions. Luckily the Enlite doesn't do that, so yay! At this point, I'm managing with a combination of increased insulin (90-120 units per day vs 40-ish) and a less carbs. I'm at <120 per day right now, and have much better days if I stick to <10 carbs before noon. My last A1C was still great @ 5.9, but it's likely creeping up through the sixes now. I'm taking this 3 hours at a time. It's been like I had to re-learn how to handle a disease I've had for 15 years, which I did not expect.  My endo team has been awesome, we have weekly+ pump updates by text at this point :)

    +1 to @TalesOfASocialIntrovert 's "try the best you can but don't be too hard on yourself for past numbers." Hysterical sobbing at blood sugar will happen, but it really wasn't your fault. Put your Endo and your pump to work, they can and will help.

  • Hi ladies! I'm so glad to see you all gravitate here. @Lafreeman21 I'm glad to see your pregnancy-in-review about control! Thanks for that. I'm in the Sept '17 BMB now. A1c 6.1, on Enlite cgm with medtronic pump. I'm learning how to make the temp basals my friend now.

  • @Sugargirl1019 Congratulations!!! I saw your announcement post on the TTGP grad thread and wanted to say congrats there but I feel weird posting as a now-lurker. But I'm so excited for you!! I hope pregnancy is treating you well so far. Way to jump on board quickly with those temp basal rates too. I've been using them recently when I suspect my rates are needing adjustment and kind of use it to "try out" a new setting for a day or two before committing.
  • @TalesOfASocialIntrovert Thank you so much! I listen to Juicebox Podcast and he talks all the time about using temp basals to control highs and how it's better to control with basals than boluses if you can act early enough. I thought to myself that I better learn how to use this to my advantage! Only 4+4, but I noticed insulin resistance from like 3 dpo and was like whaattt... haven't had this resistance since I was pregnant last time.. Can't wait for the numbers to drop ~6 weeks and to see if my A1c can get lower. Did you experience that?

  • @Sugargirl1019 Overall, I haven't had a consistant need to decrease basal rates. The only times have been the first 2 days following BFP, a few days in maybe weeks 5-7, and again last week. In those cases, I noticed constant lows despite food. Last week, I was eating 200-250 carbs a day and covering with like 5% of my "normal" bolus but still somehow fighting lows. I set a temp basal for 72 hours for 85%, which kept me pretty well in range. That's since adjusted again in the last 3 days and I'm running a 10% increase in basal rates and bolusing normal percentages. If a trend holds up for more than 3 days, I'll adjust basal or bolus rates. It's a constant battle and there are insulin need trends women generally see in pregnancy but I'm quickly learning each person is still an individual!
