Re: Ask a STM 9/28

  • Those who have switched doctors, did you feel obigated to inform the first doctor you were leaving?or did you just let the new office request your medical records and leave it at that?
  • @JannieJ I switched doctors with my daughter at 30 weeks when we moved. When they started talking about my next appointment I told them I was moving and this would be my final appointment. I took a copy of my medical records with me to the next doctor to make the process quicker.

@JannieJ depends on under what circumstances I would be moving.  If it's for a simple reason like you've moved to a new city, then I would let them know.
If it's because the doctor/office is shitty, I'd let the knew office handle it.

  • Does anyone have experience breastfeeding after having a breast reduction?  I know it's all pretty individualized and my OB basically said I won't know until baby is here whether I'm able to or not but I'd love to hear some experiences.  Even though the surgeon told me there was a risk when I had it done I was 21 and definitely not even remotely thinking about worrying about not being able to breastfeed.  Now I feel like I'll be really sad if I'm not able to even though I do know it's not the end of the world.

  • Thank you @MrsP052909  @AfKash I'm thinking of changing because even though my current doctor is super nice he is highly reccomending a c section. My second opinion has given me hope that if I want to deliver vaginally then I have a high probability of being successful. So while i havent decided which route to go yet and I know I can tell my current doctor my wishes, I want a doctor who believes it can be done successfully. In my head, more support from the doctor equals a better outcome.
  • @JannieJ What is the doctors reasoning for you needing to have a c section? Unless there is something medically that would cause concern, it is VERY early for them to already be saying you'll probably need one. Having the support of your doctor is very important.

  • @MrsP052909 with my first delivery I had a fourth degree tear. The Dr says if I tear that bad again I will likely have incontinence issues, but if I do choose to deliver vaginally he'll be sure to do an episiotomy. (Not a fan of that idea) I went to another Dr I've seen before and who has transitioned away from ob to pelvic floor specialty. He explained a lot of statistics and in my specific situation there is a low chance of long term incontinence. Even if I did have these issues, he is confident they can be resolved. It seems to me my current Dr is simply uncomfortable with my situation, and it's true, with his recommendation I will absolutely not tear, but yeah.....something I'm currently contemplating.
  • In that case, I would definitely switch!

  • @CurlyCupcake21 the two people I know that had reductions were not able to BF at all :(
  • @JannieJ I would switch. It is not in your head at all -- if your doctor is uncomfortable they are gonna pressure you to have a c-section. If a vaginal birth is important to you, go with the specialist.
  • @CurlyCupcake21 My very good friend had a breast reduction a couple years back and has no trouble breastfeeding her now 7-month-old. I feel like she mentioned that the doc said there was a risk, but she didn't have an issue. Judging from @PerraSucia's experience, apparently it really is a "wait and see" thing!

  • @CurlyCupcake21 A friend of mine had breast reduction surgery and she was able to breastfeed just fine!

  • Thanks @PerraSucia @madamerwin @jennpearl007. I figured it would be pretty individualized, I think I'm hoping to hear that more people have are able to than not but who knows. I'm not a patient person so I hate the wait and see.

  • So I have a question for moms who decided to (or are deciding to) become a SAHM after their baby was born. When did you (or do you plan to) tell your work that you wouldn't be returning? I don't want to tell them too early because I'd like to work up until at least close to my due date but I also don't want to make them feel like I left them high and dry since that isn't my intention at all.

  • @marriedhamstermom I'm wondering the same thing. I think I will mention it when it's time to plan for my maternity leave. I also want to work up until the birth (if possible), and I'd like to use my short term disability and keep my insurance until after the baby is here.

  • I'm a FTM but will be staying home after our girl is born. I just told my boss last week. He actually asked me first, but I was planning on telling him that day anyways. I think he knew it was coming though. When I started I got almost no training and it took a long time for me to get up to speed so he wants my next replacement to work with me for a couple months before I leave. I am due 2/14 and told him I'd like to work up to 2/2 and he was fine with that.
  • @SawyerRichardson I have been wondering about leaving a week or two before my due date... do you have insurance through your job? I'm just wondering if I'll be able to leave early without a doctor's note without losing my benefits.

  • @JannieJ I am completely in support of finding a provider who you feel best listens to you and gives you the information you are requesting.  However, I have read discussions amongst OBs and other pelvic floor physicians who pretty much all say preserve your continence and do a c-section after a 4th degree tear.

  • @scottipino I've heard similar things and that's what we are contemplating right now. The doctor I saw for the second opinion is amazing! *TW: He is the one who knew what to add to my fertility protocol after we went through a miscarriage, moved, and I no longer had an RE to work with. He knew what I needed when my RE whom I was paying major bucks to didn't even mention this needed addition as a possibility. TW over*

Anyways, he is always up to date on studies and he explained all the statistics for tearing again and having long term issues. We determined in my specific situation it's around a 3% chance for me to tear again AND have long term incontinence (meaning lasting longer than 3 months), which could be treated. That means it's a 97% chance that I won't have long term issues!! It's so hard to decide! It's so hard because of all the "what ifs". I mean I have heard horror stories of 4th degree tears and I don't relate to them. I was told my doctor stitched me up really well and so I have not had any issues from the tear besides being sore for a while....

The doctor also explained that a C-section isn't a save for incontinence issues later on in life. That because I've had a tear, my pelvic floor is weaker so that means as I get older I'm already at a higher risk for incontinence issues....ugh....the things we go through for our sweet babies!
  • Initially I planned on going back to work after my maternity leave. However, I quickly realized that it wasn't in our best interest for me to go back so I told my employer two weeks before the end of my leave that I wasn't coming back. I think it depends on your employer, mine was pretty flexible and at the time, the place was in utter chaos so they understood that I wanted to stay home.
  • @WinchesterGirl I do not. My office has 3 employees including myself, so it isn't offered. I'll be joining my husbands insurance starting in Jan. Sorry I can't help with that!
  • @WinchesterGirl I'm also going to be going on my husband's insurance starting in January. We did the math and while it seemed more expensive at first we will end up saving since we have to be on a high deductible plan and don't want to pay that deductible twice next year.

  • Thanks ladies, sadly I have to keep my own insurance until after delivery. Both of our companies instituted an extra "fee" for putting a working spouse onto your insurance, to the tune of $1,200 per year if we do the switch 2 months early.
