Painful cramp after orgasm?

Ok ladies, I'm looking for some insight. I don't know if this was coincidence or not. I was just sleeping, had an orgasm, and woke up immediately to an intensely painful cramp. I stumbled to the bathroom, feeling like I was either going to throw up or pass out. I got full body sweats and eventually laid against the tile once I was able to spread myself out from a fetal position. I'm 5+1 and have had brown spotting now for a week. I've been trying not to stress about it, but now I'm freaking out. I haven't been to an OBGYN yet, and I'm currently out of the country. I can't set up an appointment until I get back, so I've been hesitant to call since I'm not technically a patient yet. I've also been waiting to see if I start bleeding in earnest after this horrible cramp. But I read something on the internet that said cramping after orgasm is normal for some women in their first trimester. Has anyone experienced this? I'm just really worried right now...I don't know if any of this is normal.

  • Are your cramps usually accompanied by full body sweat or whatever else you felt? Cramping after an orgasm is normal but honestly this sounds like more than that... I'm thinking maybe a ruptured cyst or maybe an ectopic pregnancy. The biggest thing is to monitor your symptoms.  If they start to subdue in the next hour then it might have just been a cyst and not worth worrying too much about but if they get obviously worse then you might want to consider going to the ER.

    @vrj0522 This was the first time I've had what I would describe as an actual cramp. The pain/nausea subsided after about 5 minutes, and I felt better after about 10. I was still shaky, but eating some bread helped. Now, I feel the same symptoms as I've had for the past week and a half. Just a subtle feeling of pressure (wouldn't really describe as cramping, no pain) and slight fatigue. Still no active bleeding. 
  • That's really good.  I  hope it's nothing. Just because you had a painful cramp there's no need to assume that blood will follow. I would just mention it to your ob when you go in for your first appointment.

    Also I still think it might have been a ruptured cyst. Did you notice the pain on one side more  than the other? If so you might want to remember which side so you can discuss it with your ob. I've had a cyst rupture during 3 of my pregnancies and the cramp that goes with it is ridiculously painful.

  • @vrj0522 I actually noticed it was very central. The general pressure I've been feeling since I got pregnant has been on my left, which is why I remember noticing that the cramp was in the middle. 
  • But yes, I will definitely tell them about all these symptoms as soon as I get back home in a week. I may still give them a call today, just to see if they recommend anything. But for now, you've made me feel better, so thank you @vrj0522
  • I have also experienced very mild cramping for about 10 minutes after an orgasm. Nothing even near what you had though. I don't think I would be too concerned unless there is any bleeding or spotting. Maybe it would be best to avoid an orgasm for the first trimester? 






  • Just wanted to let you know that during my last pregnancy I would get cramps after orgasm every time.  It was more like my entire uterus seizing up, almost like a braxton hicks contraction.  I only remember it happening during the second trimester and later, but that might be because I'm not sure I had any orgasms in the first trimester last time (nausea and exhaustion don't necessarily get me in the mood).  Like other posters have said, without bleeding you're probably fine, although maybe mention it to your doctor when you go in (awkward, I know).

  • I had a ruptured cyst a few years ago. No orgasm beforehand, but all the other symptoms were the same. 
  • Thanks @vrj0522 So far, so good. I've stopped spotting (lasted a solid week and a half), so I'm hoping that's a positive thing. Other symptoms have been the same, but no other crazy cramping episodes. This may be TMI, but I keep having erotic dreams and I'm terrified I'm going to have another orgasm in my sleep (in case that was the actual cause). These hormones are not working in my favor right now. I'll be home on Thursday and am going into my OBGYN right away. I'm really hoping they will be able to get an appt set up for me quickly. When I first talked to them, they said it'd be the third week in October... Ugh. I'm still anxious that something could be wrong.
