Infertility

What's your protocol?

I'm always interested in seeing what protocols other people are doing. I've even used this info to ask questions to my RE (i.e. What about estrogen priming?) I was thinking it might be cool for people to post their protocols in one place.

This is my 4th (I guess) IVF session and I'm now doing a microdose lupron flare protocol with a twist (my name not RE's :)) So 40 mg of lupron in the am and pm throughout my stim cycle, added 400 mg of Clomid for 5 days on 2nd day, will add 100 mg follistim and 75 menopur on 3rd day, then (fx) trigger with pregnyl.
Me & DH - 41, DH has a cyst blocking his tubes, I'm just old.
IVF #1 scheduled May 2016 - 375/525 follistim, 75 menopur - cancelled cycle Day 6
IVF #2 scheduled July 2016 - new protocol, switcharoo...IUI June 2016 - BFN
IVF#3 scheduled August 2016 - estrogen & testosterone primed for 3 wks - ganirelix 7 days leading up to stim - 500 follistim, 75 menopur, 150 clomid , no follicle growth on day 6, stopped injectibles. Scanned day 9 just to check, some growth, 3 days 200 mg clomid, day 12 more growth, 100 follistim, 75 menopur, start ganirelix on day 13, ER = 0 eggs, ovulated to soon ( :( )
IVF#4 scheduled Sept 2016 - Microdose Flare protocol, cancelled stim day 6 converted to IUI (one dominant follicle)  :(  IUI cancelled ovulated too soon again. Taking a mental health break.
IVF#5 December 2016 - New protocol, same results. My adventure is over.

Re: What's your protocol?

  • I had an antagonist protocol. Not quite sure why! I had no Lupron, just started with Gonal-F.

    225 Gonal-F for 4 days, then added
.25 mg Certotide on day 4 through retrieval

    Triggered with Ovidrel!

-adjust follistim and add menopur as needed

    FET #2 - 11/8/16 (2 blastocysts) - Beta on 11/18
    4 more blastocysts on ice
  • I think I had your typical antagonist protocol with 4 weeks of BCPs prior to stims (would have been 3 weeks, but a cyst delayed us a week).  I stimmed with 225 iu follistim and 75 iu menopur.  On day 5 or 6 I believe we began adding in ganirelix.  And I triggered with lupron and HCG to avoid OHSS.

Do you mind me asking when you start the BCP? Did it matter? Or did it have to be on like CD 5 or 21...

  • @Zoey1019 My clinic had me start BCP in my luteal phase before I started my stim cycle.  My RE actually said it didn't matter when I started it.  After my retrieval and before FET, they had me start on CD 1-3 (I started CD 1 because I was impatient to get things moving).

  • This is my very first round of IVF. I did maybe 6 weeks of BCP? They kept me on there longer because I had low TSH levels after my first work-up. So:

    - Levothyroxone since July 17
    - Gonal-F 375 IU, 8/27-8/29, 300 IU 8/30-now (PM)
    - Menopur 150 IU 8/31 - now (AM)
    - Cetrotide .25 mg 9/3 - now (AM)

I just had my latest bloodwork and u/s this morning, and am still waiting for my next set of orders. I'm worried, though, because I need to order new medicine, but don't want to do it until I know how much I'll need!
  • @kirstencas- I have been following your journey on the other boards and I just wanted to share my latest protocol in case its something you want to discuss with you RE since, according to my RE, its the most aggressive protocol they have for low responders (and of course now I am terrified if this doesn't work for me I'm screwed :() but anyways, not sure what your plans are for another cycle, but just wanted to share in case you wanted to bring it up to your RE.

I'm doing an estrogen priming protocol (my clinic called it the Schoyer protocol). I track my ovulation on OPKs and then after that I begin estrogen patches and ganirelax during the luteal phase. After my period I start stims with follistim and we are adding in Omnitrope too (which is a human growth hormone). My nurse said they have seen this be the difference between success and failure for low responders. Its not yet approved by the FDA for infertility, so my insurance won't cover it and it costs $1200. I think we are going to give it a try despite the cost. They also want me to take 100mg of COq10 twice per day and DHEA 25mg three times per day starting now. I was already on COQ10, but wasn't sure about DHEA since I have read mixed things. I have heard for DOR, it can really help so I'm going to give it a try.

    Hope this helps!
