So I have never has an U/S done at the beginning of my cycle. I should still be bleeding pretty heavy on day 3. Do they still do the internal?? I feel like that will be really gross! Please tell me they don't do that!!
In my experience they do. Need to make sure everything looks alright before you start meds. My clinic has a sign up that you should pee when you arrive if you're there for sono. When I'm there for my baseline, I use the restroom when I'm called back instead of right away so I have the least time possible without tampon. They put down an extra pad on the exam table just in case. There will probably be some blood on the u/s probe, but it will get on the cover that they take off, and it's nothing new to whoever is doing the u/s.

Mine always does an external with a full bladder and then they tell me to go pee and take out my tampon if I am using one and they do an internal. It seems like it will be gross but it really isn't as bad as I always prepare for!
The only time I've ever had an external u/s was when they checked for OHSS and to confirm pregnancy. Other than that, it's always internal. And the ultrasound techs don't care. It's their job, so they see it every day.

I've had a bunch of internal ultrasounds while bleeding. It's not as bad as you worry about. They put a chuck pad on the table (like a puppy pee pad), and you get your modesty napkin, I mean, paper blanket, to cover up with before and after the ultrasound. And when you're done, you're left in the room with a basket of (industrial) pads and tampons, and paper towels, so you can clean up, put yourself together, and go ahead and bunch up the chuck pad and modesty napkin, if you feel shy about people seeing where you dribbled a little blood. Though to be honest, it's rare for me to see a droplet or two.
They do it all day long, so there's nothing shocking for them to see a woman bleeding.
I feel like since I've been doing IF stuff, I've completely shifted my sense of personal privacy and body stuff. I've had so many people up in my business, during all times of the month, it's no big deal to sit back, stirrup up, and scoot down to the edge of the table.

But he has brought multiple training Drs in to observe and I never see the same one twice. One time he asked if I minded if the trainee to a turn with the wand and he poked around for a bit too. Can't be shy about it anymore ...

Like the PPs had stated, they see it all the time. My thinking is, I'm might as well get used to it now because when we finally do get pregnant (hopefully) modesty will go out the window anyways!
@kfras my RE almost always does the u/s himself as well, but one time before all this TTC/IF stuff, I had an u/s for a suspected ovarian cyst and they asked if I minded if the trainee did it...won't make that mistake again! And then when I've had u/s at other places (OB, hospital, etc.) and the tech asks if I want to guide the wand in...it's like, thanks, but this is like the billionth one, no need.

You'll get used to it. I hated it at first, I was so uncomfortable and embarrassed, but it is routine for the doctor and nurses. Best of luck with your cycle!
