They typically base it on the first date of your last period. And then they confirm with an ultrasound. If you lie and say you don't know, they will do a dating scan. Otherwise they will go based on LMP and back it up with an ultrasound around 8-12 weeks. So based on your other post about short term disability, likely is they will realize you were pregnant before the policy went in to effect and you will be denied benefits. Your best bet is to be honest with your doctor and the insurance company. Fraud is no joke.
