Weekly Ticker Change Format

I was up early so I figured I'd put this together. Please feel free to start these threads as they pertain to your ticker change. For example, my EDD is 03/03/17, so I'll post in the Friday TC thread. The days of the week and correlating due dates are as follows:
Sunday: March 5, 12, 19, & 26
Monday: March 6, 13, 20, & 27
Tuesday: March 7, 14, 21, & 28
Wednesday: March 1, 8, 15, 22, & 29
Thursday: March 2, 9, 16, 23, & 30
Friday: March 3, 10, 17, 24, & 31
Saturday: March 4, 11, 18, 2& 5
Also, I scoped out a few other BMBs and the general format of the TC seems to be as below (although we can certainly add anything we want in addition):
