Anyone Skip Straight to IVF

Just curious if anyone here has done this. We have the money saved and our RE is top 10 in the country for successful IVF. I just don't want to waste our money resources and sanity on rounds of clomid/IUIs. Obviously this all revolves around talking to him first which should be this week.

    I personally did not, but it is your treatment, your body, your choice. It may be recommended that you try IUIs first, but you certainly shouldn't have to.

    Edit: it might also depend on what your diagnosis is. If it is MFI, then I don't see why they would make you do IUIs.

  • We went right to IVF. Our issues were that my husband has 0% morphology. RE said we could try IUI but he wasn't hopeful that it would work. We did IVF with ICSI and just found out Friday that we're pregnant. We did PGS as well b/c we wanted to give ourselves our best chance, especially since DH had 0% morphology, we were concerned that there could be some chromosomal abnormality if we didn't. 
    Me: 33 (no known issues)
    DH: 33 (lower motility; 0 morphology)
    TTC since Oct 2013
    IVF round 1 January 2016 (w/ ICSI)
    ER: February 17, 2016; 15 eggs retrieved, 14 mature, 11 fertilized
    Day 5 Blastocysts: 8 sent for PGS - 5 Normal
    FET date: April 12, 2016 - DS born Dec 7, 2016
    IVF round 2: FET : Oct 4 - 1st Beta 188!

  • I only did 4 months of Clomid before moving on.  They were going to try femara but I asked to just skip it and go for IVF.  I'm not getting any younger. ;)

    Tried 3 rounds of clomid with Ovidrel trigger (BFNs)
    May/June 2016 IVF   6/1 ER 7 eggs retrieved, 7 fertilized via ICSI  6 embryos made it to blasts.
    6/6 2 AA embryos transfered, 4 BBs frozen
    Beta 6/18  -- BFN
    FET #1 August 22 BFN
    Dec 2016/Jan 2017 Round 2 was a bust. 6 eggs, Only 1 embryo made it, no transfer
    FET #2 cancelled 10/17 due to uterine polyps. Postponed until 9/2018 due to hip replacement.

    FET #3 9/11/18 Uterine scratch and antihistamine protocol/lovenox done this time. 2 AA hatching blasts put in.  BFP 9/20/18!! 

  • We jumped straight to IVF for the same reasons you list, plus AMA. We didn't want to waste any time and knew we'd want PGS. RE and OBGYN recommended IVF as well.
  • Severe MFI & low AMH for myself. We skipped IUI's & went straight to IVF w/ ICSI. We were lucky with our 1st fresh cycle attempt. GL to you!

    Because I had good insurance coverage  (once ivf was approved), I wanted to use the insurance.  Unfortunately despite the fert. doctor saying that that was likely the only way I'd get pregnant, the insurance company still required meet the requirement of 3 IUIs. Since my ob/gyn told me my egg supply/quality was borderline low after we had been TTC for a month or two (saw the dr because I was having  irregular bleeding and other symptoms...actually thought I was pregnant before irregular bleeding started), I was referred to a fert. dr right away since I was almost 41. Insurance still made me wait a year and do the IUIs. Now that I'm approved, the coverage has been great.
  • We did one round of IUI before skipping straight to IVF because we were paying OOP. We didn't want to take the chance that we could have been successful for $750. After one round though, we knew the chances of success were so much higher we went to IVF.

  • I really wish we went straight to IVF, but unfortunately we didn't have a doctor at that point in time who could make the best recommendation. Instead, I went through 7 months of clomid with my OB, then 3 IUI cycles with a previous RE and now doing my first IVF cycle. It really does depend on your personal situation. Sure, IVF will give you the best chance for success, but it's also the most $$$ option. Then age, medical history, etc come into play. See what your doctor says when you meet with him as well. If he's truly one of the best, I have no doubt he will take all the factors into consideration and make the best recommendation for you. Good luck!

  • We have MFI and definitely would've gone straight to IVF if we didn't have insurance coverage for IUIs and no IVF coverage at all.  Even with the insurance coverage I feel like we're wasting our time with IUIs.  It is a completely personal choice though.  Good luck to you!

  • We are skipping straight to IVF w/ ICSI and PGD cause of a genetic disorder. Luckily we have amazing insurance coverage so it's covered 100%!
  • We were originally planning to skip IUI but the RE ordered a Clomid Challenge to test how I respond.  We figured why not do IUI (even though I only have 1 working tube) since I would already be taking medication.

  • I didn't skip straight to IVF, because we have unexplained infertility. So, there was nothing completely ruling out IUI being successful. A friend of mine had to go straight to IVF due to extremes MFI and she is now 4 months pregnant. I wish we had moved straight to IVF, since I feel like I wasted a lot of time on Clomid with my OB. Luckily after my second failed IUI, my RE recommended we move on and not keep wasting time. So I'll be starting BCP in a week or so once AF comes and hopefully ER will be in June!

    I'm about to start IVF after only one try of clomid. It's a personnal choice, I just find the wait too hard and I didn't believe enough in IUI. I mean I know it could have worked, but the stats are not encouraging. I have a max lifetime insurance and I wanted to spend it on something that gives me more chance of success. Also I've read so many time " I wish I would have done IVF sooner". That convinced me!
  • We went straight to IVF.  The stats didn't look so good for IUI.  It is all out of pocket for us, and I didn't know anyone personally who had been successful for IUI.  Our RE recommended we go straight to IVF.  I'm glad he did.  We were successful with our first fresh cycle, not successful with a frozen cycle, and going to do one more fresh cycle to see if our little girl will have a sibling or not.  Best of luck!  Make the choice that feels right for you.
  • @stranzm good luck this round! I think we have decided to try on more round of IUI once we can get back into everything. If that doesn't work we are going to talk to the doctor about moving on. 
  • We did too many rounds of Clomid/trying naturally bc we didn't know we were also dealing with MFI. Once that got diagnosed, we jumped straight to IVF. Best decision ever, my son is now 7 months and we didn't have to screw around with IUIs
  • Yeah, we also have MFI, and unexplained for me, so IVF made most sense for us.

  • Girl I have been really wondering the same- I am unexplained and "young"- 30- so it seems general protocol is to keep trying IUI. I'm about to do my first and while I am a tiny bit hopeful, I understand the statistics, so I am pretty skeptical. I don't know if I have it in me to do 3 or 4 IUIs with such low chances. I spend a lot of time feeling impatient and just wanting to skip ahead.  
    I go back an forth on it, which tells me I'm not ready to make that jump yet.

    Good luck with your first IUI! Hopefully it works and you don't even have to think about this!

    We did one iui before going to IVF. We are out of pocket and have an unexplained diagnosis. DH is 44 and I'm 39. We figured the statistics were better if we did IVF instead of another round of Iui. Do what feels right for you.

  • Hello, MMD! I'm sorry you're facing conceiving difficulties. And actually I've always sympathized with people being in the same boat with me. :( I understand your worries especially when there is the money issue so we have to make the right decision or pay double price. Years ago I was diagnosed on PCOS and endo. DH is absolutely ok. Well they did suggest us to pass rounds of IUI as the less invasive option before moving on to the more complicated IVF. But actually the only thing we did was a lapo. They said it might improve our position. And I was so disappointed when it all turned to be in vain. Our surgery was a complete disaster causing left ovary scarring. After we went oversees to Ukrainian center of human reproduction. There they gave us only 5% verdict of conceiving with OE. So we moved on with IVF using donor eggs there.
    I'm not sure what should I advise in your case. Probably you should wait for the expert's opinion after your medical records studying. Will be thinking of you and wish you good luck  :*
  • I have PCOS, and tube issues. After surgery to remove my right fallopian tube and "clean up" the left, my RE actually suggested skipping straight to IVF. She said given my PCOS, recent surgery, and age she would suggest going straight to it. So I asked her given those same factors, and if we wanted to try on our own how long would she say would be safe, and she said 3 months. So, after my full recovery, we are starting 3 unmonitored cycles in July then going straight to IVF. Everything happened so quickly for us after being referred to the RE that I honestly, just wanted to enjoy a few months off from the appointments and blood work. Plus, if we can be successful with just the medication, that's even better. But I have a friend who went thru 5 years of IUI and monitored cycles before her RE would even suggest IVF...

    Feb 2016: 1st RE apt - diagnosed with PCOS still possible uterine septum. Put on BC for cysts, HSG will be in March

    March 2016: HSG showed both tubes blocked (right tube holding fluid), no uterine septum, need surgery

    April 2016:  Surgery to remove right (hydrosalpinx) fallopian tube and adhesions from the left - all went well!

    May 2016: Waiting to start 3 months of un-monitored cycles in July with metformin and letrozole. If no success, going straight to IVF!

    The only people I know who have really had success are lesbians with no infertility problems (except that they can't inseminate at home).  I've seen people online who have used IUIs to get past infertility issues, but I don't know people personally in that boat.
    Me- 39 (turning 40 in April), TTC for the first time ever (since Jan 2015), low ovarian reserve
    Married 3/14/14 to my wonderful wife, but her sperm count is rather low
    TTC with frozen donor sperm and science

    7 IUIs, 7 BFNs.
    2 IVF attempts, both cancelled and converted to IUI, both BFNs.
    Decided that my tired old ovaries are ready to retire.
    Next step- reciprocal IVF, using my wife's eggs, my uterus!  
    fresh 5 day transfer (2 embryos) 4/17/17- BFP! 
    Identical twins "due" 1/2/17 (but anticipated arrival sometime December)

    I jumped straight to IVF as IUI didn't seem to be the best option for me based on my diagnosis. We got pregnant on our first Frozen Egg Transfer. I really think it is going to depend on your specific diagnosis and what your RE thinks is best. I had a very low egg count and when I did ovulate the eggs weren't mature enough to sustain a pregnancy because I would ovulate so early. She basically said I had the same odds of getting pregnant with IUI as I did on my own with was less than 2%. My RE felt like IVF would give us the best chance because everything is so controlled and she was right. Ultimately it comes down to what you want... I didn't want to go through any more failed attempts and disappointments because I felt like if I did that I would be to wore out emotionally and physically to even move forward with IVF. Also, I was 33 at the time of my diagnosis so my time clock was ticking because every year I waited my egg count dropped and at a faster rate than someone 30 and under, so I didn't have a lot of time to even try to see if IUI would work. Then I got pregnant naturally before we even got a calendar for my first cycle and that pregnancy ended in miscarriage because the egg wasn't mature enough. That was def the big confirmation that IVF was the only and last option for me.

  • My OB/GYN said to head straight to a specialist, which we did. Once we had all our tests done, the RE told us to go straight to IVF, as it would be virtually impossible for us to get pregnant without it due to multiple factors (mainly age). I start my meds in June and ER is set for mid July. Good luck to you! xo

  • DH and I are seriously considering jumping to IVF if IUI#2 turns out to be a bust. It's becoming too much for us, and with our low motility rate I'm just not very hopefully that IUI will work for us.

    @wintersong555 that is what my husband and I are thinking as well now.  If IUI #2 (hopefully in June) doesn't work, we are going to take a break for a month as we are traveling in July and then meet up with our RE to discuss getting on the waiting list for IVF.  We just really don't want to dump a bunch of money into failed IUIs.
