Blood clot risk experiences??

I was curious if any STMs or people who already delivered this time around had any issues with blood clotting situations.

I have the factor V leiden gene, which makes you more likely to get a blood clot. However, i had a weird high blood pressure related hospital visit last week (no pre-e but suspected.) and thought all the results were in, but turned out another one came in today. It turns out i have something called low antithrombin. I figured it was ssomething caused by my factor 5 leiden. But of course nervous pregnant lady couldn't let it go and fell down the doctor google hole.

It turns out that low anithrombin and factor 5 leiden are definitely different things, and when you have BOTH of them its a ''significantly'' increased blood clotting risk. So i called the midwifes office who said they too just got the results in, so i asked them to please not forget that i have the factor 5 leiden and definitely give me a call once they are able to go over them today. Which they said they would do as soon as they were able to.

I am midly panicing that i am going to get tossed into an induction cause of this. I am super curious if anyone has dealt with any kind of blood clotting related risks in late pregnancy and how your care providers handled these risks. Hoping that maybe i will end up on a medication instead of an induction, but definitely just feeling a little concerned right now, but trying to keep myself calm as I dont need my wonky blood pressure to decide now its also time for that to kick up!

  • I had a tiny blood clot in my lung in my early 20's that puts me at a higher risk of a repeat clot now. Because of that I'm on twice daily injections of heparin until I give birth, then an injection of tuberculin for about 4 weeks post-partum. 

    It doesn't affect anything regarding labor, the heparin is only in your system for 6 hours, and there is an antidote for it should labor happen quickly. 

    I will be induced to size of the baby, not blood clot risk.

    Good luck, if they do medicate you and you need to give yourself injections they aren't that bad.

  • Yes I'm a blood clot risk too. I have too many platelets. I did end up getting induced because baby had IUGR and was small. I'm now on injectable blood thinners (Fragmin) for 6 weeks. 

    If your condition doesn't affect baby or baby's growth then they shouldn't need to induce you (and it's not the worst thing in the world if they have a medical reason to). Usually the biggest risk is to you during the post partum period in which your body is in a prothrombotic state (which means it's more like to clot). It sounds likely you might end up on some blood thinners after birth as well but I'm not very familiar with either of those conditions. Either way, dont stress about it. Your doctors will make sure you and your baby are safe and well taken care of!

    I have a high D-dimer count and have been on heparin for about 2 months now. What I was told was that 1) it won't harm the baby, it will actually increase it's ability to be well-irrigated, 2) it won't affect breastfeeding. Now the downside is that depending on the formula, the latency period might be too long to be able to have an epidural. So if you're being prescribed heparin, and an epidural is something that you want, then don't forget to mention it. As for induction, my doctor mentioned it as a possibility but only because during my last visit her head was measuring quite large.
  • They are going to talk to one of the midwifes and see if they want to push up my appointment and consider options. But the girl i talked to on the phone wasnt too too concerned, she said its still within a normal range of being low. I dont really know what that means but i guess just that means that its not low enough for them to be concerned. However this is the ''nurse'' type girl there (I dont know what her actual title is) and she is still waiting to actually talk to the midwife.  

    Sunday is 37 weeks, i just want to make it to then!! 
  • I have a history of blood clots and received heparin after delivery every 12 hours I was in hospital. I don't recall getting anything else for it before though. 
  • The midwives office doesnt seem as concerned as googling made me. Which i guess is typical of over googling things. The swelling in my hands and feet is so bad and ''foreign'' to me that i think I might just be kind of associating the two and getting worried unnecessarily. She said it was okay to wait until my appointment thursday to discuss it further but no need to rush in sooner.

    I have never have any presenting symptoms of the factor V leiden. My dad didnt even know he had it until he was in his 60s and i only know cause of that episode that he had. However i was constantly warned by doctors when i was a teenager ''it probably wont be anything to even be concerned with late until into a pregnancy'' so of course its on my radar now that im in said stage ive been warned about for 10+ years.

    I have been on blood thinner (lovenox) my whole pregnancy and will soon be switching to heparin because I have an history of blood clot. Have you consult with a doctor prior your pregnancy regarding the factor V? My OB makes everyone with any clotting factor problem meet with a High risk specialist to determine the proper plan of action. Some only need baby aspirin, other only post partum coverage( where the risks are the highest) etc. 
    I don't want to sound harsh but midwives aren't doctors and they deal with regular non complicated pregnancy, so maybe a referal with a hematologist would be the safe thing to do. I really don't think your baby is at risk but the post partum period is the most critical one for clots and you definitely don't want to deal with a DVT or PE with a brand new baby. 
    Meanwhile for your swelling, have you tried compression socks, Epsom salt bath ( not too hot) and increasing your fluid intake??
