No response on Letrozole 7.5mg CD11

Hi Everyone,

This is my first time posting. I have PCOS and we started trying a few months ago.  Started on 100mg of Clomid, no response, 2 16mm follicles on 150mg, O'd with Ovidrel shot, BFN.  After that the doctor put me on 7.5mg of Letrozole, CD 2-6, and I went for an U/S CD11 and had no response at all.  They scheduled me to come back for another U/S on CD16.  I am under the impression they are going to want to speak to me about moving on to injectable medications at that time, but I was wondering if anyone has had a similar situation and went back for an U/S to find out the Letrozole had worked after waiting a few more days.  Also, any info on experience with injectables would also be appreciated.

  • Hi @dpizzurro - coincidentally, I'm going through something similar now. After 3 cycles (see my signature), I went for an u/s this past Saturday at CD11 and the nurse said it looked like CD5. I'm going back in the morning to see if anything has developed. I'd be interested to hear others' experiences, too! I'm also curious about injectables -- cost, protocol, etc. -- as I have a feeling I am going to be on that route soon. GL to you!

    Frustrating stuff! I hope that thinks kick up a notch by the next ultrasound date. Are you on Meteformin? That seemed to make a big difference for me in addition to the letrozole/fermera at 5mg (with all of that I ended up getting a 27mm follli). 

    Might also look into adding acupuncture to your list. Sorry no advice about injectables other than trigger shots. Best wishes to you!.

  • Thanks for your replies.  It seems I haven't run into many people who have been prescribed 7.5mg of letrozole.  The fact that I haven't responded to it makes me feel like I'm in pretty bad shape.  Sucks.  I'm not on Metformin, I don't think the doctor thinks I need it at this point.  He said I'm a classic case of lean PCOS, whatever that means.  Doesn't seem like anything else is working at this point, so I'll mention it when I see him on Thursday.  Also, thanks for the acupuncture suggestion, a few friends have suggested it as well.  Maybe its time to look into it.   • I started with a low dose of letrozole. We also had no response to begin with but they raised the dose and the response was much better. From my understanding, this early in the cycle they are just trying to feel out how much you can take. I had absolutely no response to clomid. My third try was with clomid, then back to letrozole. We had the most success with a hybrid cycle of letrozole and gonal F. GL.

  • Unfortunately they started me on the highest dose of letrozole they will prescribe, 7.5mg, so the doctor didn't want to cycle me on it again since I didn't respond. So it looks like some combination of injectables will be the next step. I'll know more tomorrow. 
  • @dpizzurro Since you responded to 150mg of Clomid, why did they take you off of it? Sorry if I missed something.

  • I guess they felt I didn't respond as well to the Clomid as they had wanted me to and it was at the highest dose so they wanted to try something else.  That is my assumption anyway.  Seeing as my follicles grew to 16mm and I know they like to see them between 18-30mm.  
  • @dpizzurro I see. I wasn't familiar with Clomid dosages. I do hope injectables work for you. I had a brief conversation with a nurse about them this morning at my RE's office, since I'm approaching the time (6 cycles) when they usually move on from oral meds. 

    Also, I went in for another u/s today on CD 15 and had a 17mm follicle. I am going to trigger tomorrow. So there is hope that the Letrozole may work for you after a few more days! I hope it does!

  • dpizzurro Sometimes they worry about too many follices maturing (resulting in multiples) with Clomid...maybe thats why a switch...so they could do a higher dosage of Letrozole with out worrking about more than 1 or 2 dominant follies.

  • @dpizzurro I did a 'stair step' of letrozole. started low at 7.5 and bumped up every 5 days - I did trigger and O that cycle + TI; BFN but at least there was a response with that method!

  • Went back to the doctor today and confirmed I had no response to the 7.5mg of Letrozole this round.  Found out he switched me from Clomid because it was thinning my lining.  He wants to try another round of the Letrozole at the same dose and have me back for another u/s Thursday.  He said we could try Clomid again after that if I want.  He is hesitant to start injectables because of the lack of control they have in regards to multiples and would prefer IVF if it comes to that.  We shall see. 
  • I'm sorry to hear that, @dpizzurro. That sounds very frustrating. Maybe there is something that you could take with Clomid to help with the lining issue. Keep us updated!

  • So after second round of Letrozole at 7.5mg I responded.  I have a 20mm follicle in my left ovary and a 23mm in my right.  Had the Ovidrel shot this morning.  I'll let you know the outcome in a couple of weeks.  Fingers crossed. :smile:
