Still hope?

I went in for my first u/s yesterday at 5w5d.  My HCG at 5w4d was over 7000.  The only things visible on the u/s was the gestational sac and the yolk sac.  The u/s tech said the gestational sac was measuring at 5w1d, so it is normal not to see a fetal pole or heartbeat as of yet.  This morning i get a call from the nurse at my OB office, she tells me to prepare myself for a miscarriage as there should have been a visible fetal pole and heartbeat with that hcg level.  The nurse also states that the Dr. will not be ordering any further HCG test and she will have to talk with him to see if he will order another U/S.  I have been feeling fine besides tender breast and fatigue.  I have had no spotting/cramping/bleeding.   I am flabbergasted that i would get a phone call like this that would add extra stress when nothing is for sure.  Has anyone had a similar situation?  Can this still be a viable pregnancy?

    My HCG was 27334 (yes 27334, it's twins) at 5w5d, all that was visible on the ultrasound was the gestational sacs and yolk sacs in each. Today (not sure of current HCG but last check on Friday was 40000) I had two tiny little fetal poles with two strong little heartbeats. I certainly wouldn't count yourself out yet...push for another ultrasound in a week or so!

  • At 5w 5d my HCG was over 4000 and they said it was too early to see a heartbeat. All we saw was the sac and my doctor felt like it was fine at this stage. I have another next Monday at 7w 2d.

  • I don't have info about the #s, but it's definitely fine to not see a heartbeat before 6 weeks.  I would definitely tell your doctor about the phone call next time you go in (or if you call).  I can't imagine it's good practice to have a nurse calling people telling them to prepare for a miscarriage.  If nothing else, that info should be given in person.  And your situation definitely doesn't seem like one in which miscarriage is imminent.

  • If you're not bleeding or cramping, I'd try not to worry about it. I've never heard of nurse making a call like that. I can't believe she put that in your head! It's completely uncalled for. Side not: I'm baffled by all the blood tests and u/s. My doc won't do any of that until 7 weeks at the earliest. 
  • I'm not a doctor but I can confirm that other people I know have been through that. They continued to have a normal pregnancy. So is there hope? I would say so. 

    (You guys are all lucky btw where I live a doctor won't even see you until 10 weeks... no chance for an ultrasound or anything at that time)

      

  • I had an ultrasound today at 5w4d and they found a gestational sac & yolk sac but no fetal pole. The dr was not worried, nor was the nurse or the midwife I spoke to afterwards. It's still very early, and I was told that this is totally normal. There is certainly hope!! What an awful thing for a nurse to say. I was told by the nurse to prepare for a miscarriage when my hcg was only 84 at 15dpo. It more than doubled two days later, and was 1814 a week later. I will never understand why some people would tell someone to prepare for a loss that isn't currently happening.

  • I also think that's a wacky assessment. I had an u/s at 5-6 and Doctor was very happy with gestational sac and yolk sac. Said he expected to see heartbeat in the 6th week. Also, due to later implantation, sometimes we're not as far along as the doctors or we think we are. At this early stage a few days makes a big difference what you see. I'm a fan of waiting to freak out until there is something to freak out over. Hang in there mama! 
  • I am not super familiar with Hcg levels, but I feel like that's a normal US finding at 5 weeks.

    Also, if you don't have bleeding in the next week they should definitely be willing to offer you another US and hcg level.

  • So I was in the boat.  From LMP, I should be 10 weeks.  I went in at *8 weeks, no heartbeat, only a sac. I freaked out because I've had a blighted ovum. Well I went for a dating and viability u/s.  Turns out I am now only 7 weeks (we saw our peanut and heartbeat, 146 bmp). I was only about 5/6 weeks when I thought I was 8. Definitely hope!

  • Absolutely and I would be extremely cranky about that poor commentary. HCG levels vary greatly from person to person and at 5+1, having a yolk sac is perfectly fine. I have done a lot of reading on this topic and it seems that the measurement of the sac & doubling time that are most relevant. Measuring a couple of days early can be due to later O. We aren't machines. 
  • At 5w6d I was bleeding and all they could see was a yolk. 
    I got told not to worry as it still could go both ways. Pretty early for a heartbeat to show in week 5.
    At 6w4d I had a new us, suddenly there was a heartbeat and my snowflake had more then doubled in size!
    Not sure what my HcG was around that time, here they say HcG levels are quite individual, so they don't look just at that.
    Don't give up hope just yet, I'll keep my fx for you!

    OMG same!  My last pregnancy was HCG testing followed two days later by more HCG testing to check for doubling, urine/bloodwork etc. And then an US at 7w.
    This time (new OB)...I was told not to come in till close to 8 weeks. I had just a pap and an US by the Physician's Assistant...no urine, no blood...didn't even see my doctor! Baby measured 6w2d and by my calculations should've been 7w3d but the sack and baby are in proportion and we could see the beat. It was in the 90's which I Googled like an asshole just now to see it's "low" but the tech said that at 6w that's totally normal. I have to go back on 4/5 for a repeat dating US (and still no blood/urine...wtf!?). If baby is indeed 6w2d then i'd be 8w on the nose for that appt. I feel like they are pretty lax considering i'm 42 and had a velamentous cord insertion and induction due to heart rate decals last time lol. Trying not to spazz about it.

    @jolenef07 I was told I probably ovulated later in my cycle due to measuring a week behind. She said it's pretty normal and about 40% of the time the baby measures smaller on the first US.
