  • For the PGS testing (looking at chromosomes), our geneticist told us that a 35 year old woman't embryo is about 50/50 for being chromosomally.  It goes up as you age (I am 38, but my ovaries are aged to ~50 from my chemo, so my rate is now much higher). They say that on average, for every child a woman has, she has also had a miscarriage that she never knew about--miscarried on day 1 or 2 and never knew she was pregnant.  This because of chromosome issues.  Everyone knows of Down syndrome, which is one of the few chromosome issues that IS actually survivable/compatible with life.  Most won't even implant correctly.  

    When you do PGS, they make sure that the embryos you are implanting actually have a chance to become a baby, whereas the abnormal chromosome ones will not.  Of my 2 embryos that made it to freeze as blasts, one was 5AA and one was 6AA.  Beautiful, perfect looking embryos.  But the 5AA one on PGS testing has a chromosome problem.  No matter how beautiful it is, it has zero chance of becoming a baby someday.  

    PGS testing is going to become absolutely standard in the future.  My RE says that even for a 45 year old woman, they would only put in one embryo if it has been PGS tested.  The higher rates of chromosome issues as you age are one of the main reasons IVF rates are as low as they are.  So many women are putting in pretty embryos that have no chance of sticking, because they have an unknown chromosome issue.  Totally worth the $$ to me to make sure that you are spending all this time and effort and money to put actually viable embryos in.

    Also starting ER in June (my sister is donating eggs), and will try for a boy/second pregnancy
  • Thanks for the chart @Drove2u!  Not sure what they determine as favorable vs unfavorable, but maybe it has to do with the grade?

    @Knottie1456036854 - Thank you for all that info!  PGS sounds absolutely like it's the way to go.  I feel really good about spending the extra money to do it.  We didn't really ask my RE how much extra it would be to do it, we just went with his recommendation and then he had us talk to the clinic's financial advisor who pretty much just showed us the package we had selected (called the ONE plan).  Basically the package costs around $14k which includes everything except meds (appointments, ER, ICSI, PGS, and FET of one embryo).  Each additional FET is about $3k.  I have no idea how much cheaper it would be without the PGS testing but I don't care because I know how important it is.

  • @wifeinraleigh28 That's about what we are paying. Package is $10,000 and PGS is about $3500. In terms of fibroids, I've had 25 removed over the past 4 years. I have six that they can see now ( and my last surgery was less than a year ago). I will likely need a hysterectomy in the near future. So, we're implanting two after PGS.

  • @Drove2u we had the same chart given to us too. And I will also be 35 soon and I'm tired of wasting time. I had 6 failed iuis and I'm ready for something with better odds and if two embryos take they take I have heard people who put two in and only one takes. I'm up for the risk and will be happy with whatever the results are! Good luck

  • Hi Ladies! I would like to join the group. I am 36 my DH is 41 and our dx is MFI due to partial retrograde from testicular damage from a MVA years ago. I have 3 ****ments*** older children from previous marriage. All my labs are great Amh of 3.1 but my AFC is on the lower side, which is around 8-9. We are doing embryo banking because of this. I had a retrieval done in February and had 9 eggs, 5 mature, all fertilized 4 on day 3 and 2 on day 5 but only 1 made it to freeze so we have 1 5bb so far. My next retrieval will be around April 8th and depending on how many we get we might do another retrieval in May. Hoping to have 6 embryos banked total. I have baseline us and labs Thursday and stims start March 28th. I will be on a short protocol of gonal-f, menopur, Hgh and cetrotide with a dual trigger of hcg and Lupron.

    Me-36 healthy

    DH-41 MFI due to Retrograde

    1st IVF 2/2016, 2nd  Ivf 4/2016 then banking w/PGS and Fet 5/2016

  • Thanks so much everyone!  I emailed the nurse at my clinic in charge of the Megaset trial (which my RE told me to do), and her email back to me made it pretty clear that I could still not qualify though following my CD 3 labs.  This was her email: "I have put orders in your chart for the day 3 labs. You can come in on day3 and get those drawn. Once I have the report, I will let you know if they are acceptable to proceed and will let you know what the next steps are."

    If my labs aren't acceptable to proceed I'm afraid that my original plan will just get pushed back further.  Oh well, I suppose it's worth it to do this if there's a chance I will qualify for this trial.  Keeping my fingers crossed.

  • Good luck @wifeinraleigh28.  I had my day 3 labs today to get them out of the way.  I don't have the option for a clinical trial because no docs in my city participate.  I hope you get a chance to join the clinical trial where you are.

  • @ultratomato The megaset trial is a research study that compares two medications used for controlled ovarian stimulation in IVF.  It requires that you do ICSI and PGS testing, and a fresh transfer of 1 embryo.  Because PGS testing takes awhile for the results, you do the fresh transfer before the results of the PGS testing come back.  Eligibility requirements are that you're between the ages of 21-35, have regular cycles between 21-45 days, have an AMH of 5 or higher, and a BMI of 30 or less.  Apparently I also have some other labs to pass on CD 3, but I don't know the details.  I'll certainly share with you ladies once I know find out more about how the study works.  @Knottie1455746131 You're right, if I get to participate it is like winning the lottery!  Really hoping everything checks out on day 3 labs!

    Hello! I'm 27, DH 26. We are dealing with only male factor. Very low quantity, morphology, etc.. Doc says that everything looks perfect on my end. I am on bcp right now and I am expecting to start stims on April 20th. My doctor only does FET so I am hoping to have my ET at the end of may or early June.

    This is my first time going through this and I am very nervous. It is nice to know there are other ladies going through it around the same time as me.

  • @chickyclg Yes!  Of course I WANT to be under for this, but never like being under anesthesia.  Also not looking forward to the recovery but I also know that I'm in good hands with my DH.  And it will all be worth it in the end so I am absolutely willing to go through this.

  • @wifeinraleigh28 I hate going under anesthesia.  Have Gatorade just in case, it really helped my recovery.  My RE predicted the day of my trigger that I would unexpectedly develop OHSS so we had a last minute change in protocol which was fun.

  • @chickyclg We are doing ICSI. My ER says that because of how good my tests look and my family history we have a really good chance of being successful. You never know though... That is what I am nervous about. I hate the idea of getting so excited just to have everything fall through.

  • @chickyclg Thanks for the gatorade tip!!  My RE also warned me about OHSS as a possibility.  Hoping that won't happen but I know it very well could.

    @Kerami1 The original plan was to start stims on 4/9 but now that I'm likely going to be doing the megaset trial I have no idea what the plan will be.  I have no info about it, other than I have to get day 3 labs done once I get AF to make sure I'm eligible for the trial.  Then have to follow protocols of the trial.  It's actually making me really anxious not knowing what the *plan* is.  I'm just trying to relax myself and accept that I have no control, and to just be thankful that this trial is even a possibility.

  • Did anyone else have to do a zpack and bloodwork for hiv, hep c, and etc? My DH and I had to do both.
  • @ultratomato we did not, at least not that I am aware. But I think it is a very responsible thing! My dh and I both did std screening before we did the deed the first time, I was not a crazy college girl by any stretch of the imagination but just to know...

    Embryo transfer scheduled for April 28, 2016 and beta test May 9, 2016 (day after Mother's Day!)
    Transfer Meds include: Lupron Depot (4/1), Minivelle Patch (every 3rd day), Estradiol (3x daily), Amoxicillin, Progesterone in Oil, Methylprednisonlone. Lovenox and baby asprin added after transfer. 

    3/22/16 - Sono Saline ultrasound cyst to be aspirated on 4/1/16 if not cleared up by 3/29 US - It cleared on its own

    @ultratomato We both did the test for STD's and I had genetic carrier testing (cystic fibrosis etc). They took so much blood, I didn't think I'd have enough left!

    I have to do CD3 tests again! Because my last one is now 6 months old. We also are doing a sperm DNA fragmentation test and should find out the results of that in a couple days.

  • I just picked up 5 prescriptions, ugh, and these are the regular pharmacy things.  Looks like I'll be taking Estrace instead of bcp from 4/4 - 4/20 and then starting stims.  Doing a trial transfer on 4/16!  It's getting real now!

    Does anyone have any experience with taking estradiol (Estrace) instead of bcp? So far, from what I read, it looks like something called estrogen priming.

    IVF #1 scheduled May 2016 - 375/525 follistim, 75 menopur - cancelled cycle Day 6
    IVF #2 scheduled July 2016 - new protocol, switcharoo...IUI June 2016 - BFN
    IVF#3 scheduled August 2016 - estrogen & testosterone primed for 3 wks - ganirelix 7 days leading up to stim - 500 follistim, 75 menopur, 150 clomid , no follicle growth on day 6, stopped injectibles. Scanned day 9 just to check, some growth, 3 days 200 mg clomid, day 12 more growth, 100 follistim, 75 menopur, start ganirelix on day 13, ER = 0 eggs, ovulated to soon ( :( )
    IVF#4 scheduled Sept 2016 - Microdose Flare protocol, cancelled stim day 6 converted to IUI (one dominant follicle)  :(  IUI cancelled ovulated too soon again. Taking a mental health break.
    IVF#5 December 2016 - New protocol, same results. My adventure is over.


  • @ultratomato We both had to do standard IVF bloodwork which included STD testing. I haven't been told anything about antibiotics yet but if I do at least I'll know it's common practice!

    @ultratomato After our initial consultation with my RE she ordered the standard bloodwork panel for HIV/STDs/etc and to see if we're up-to-date on vaccinations (the bloodwork was also how we found out that I'm a carrier for a single gene disorder that makes IVF necessary.) My insurance covers IVF, with one of the requirements is being up-to-date on certain vaccinations, so I had to subsequently get the chicken pox vaccination - even though I had it as a child! lol

    I was given doxy to take pre- and post-HSG, and I think I will be given it for the ER as well - we are doing PGD/FET so they said it won't be an issue (since I guess doxy is a big no-no when pregnant?)...my RE said that it's usually common practice to give antibiotics any time they insert anything down there...
  • Well I spoke with a nurse at my clinic and found out what my new timeline will be if I qualify for the Megaset Trial. Once my cycle starts (hopefully within the next week) I will do CD3 testing.  If everything checks out okay for the trial, then I will sign consents and do even more testing, including a repeat blood test because the trial wants to do their own AMH test to verify it is over 5 (I guess they don't trust other labs).  Basically I still have a lot of hoops to jump through before it's a go.  If everything checks out fine for the study at that point, then I will start stims with my next natural menstrual cycle on day 2 or 3.  So I will likely not be starting stims until the very end of April.  Retrieval likely sometime in the first couple weeks of May, and fresh transfer thereafter.  Participating in this trial does move things back some, and then there's also the chance I still won't qualify, which means my original plan will be pushed back as well.  Right now I'm just hoping for the best and trying to relax!  Not so easy for my type A personality.

  • @wifeinraleigh28 Lots of hoops but still so exciting to be moving forward once your cycle starts and/or hopfully getting a firm date on when to start stims! FX that their AMH test has the same results as your clinic's! :smiley:
