24 hour urine collection

So went for my routine 2 week appt yesterday and got sent home with a jug to pee in for the next 24 hours and bedrest until atleast Monday because of my bp. It's been "normal" the whole pregnancy and was 140/90 yesterday,  but the in office test didn't show any protein in my urine. Anyone else going through this or been through it before? Wondering what the possible outcomes are from this point and if it's possible everything is okay and I will be able to make it to close to 40 weeks, fingers crossed!

  • To be diagnosed with preeclampsia you have to have protein in your urine and a BP at or greater than 140/90. They want to check your urine over 24 hours to see if it's above the threshold which I believe is 0.3. If your 24 hour test comes back and your BP stays under control your fine. Generally they only have you do the test if your BP is over 140/90 on 2 separate occasions. Good luck.

  • I had to do it about week 17 and it came out with no protein in my urine. My bp is just running high, so I have to monitor it daily. Just because they have you do the test doesn't mean you need to worry!

  • I had to do multiple 24hr collections before they would officially diagnose pre-e. If this one comes back with no protein and your BP stays at 140/90 or goes lower, you are in the clear. If your BP stays elevated, but no protein, then you may just need to monitor the BP. If protein and high BP, your OB will probably have you repeat the 24hr collection while monitoring your BP and do a quick NST (non-stress test) and maybe US on the baby and placenta just to make sure everything is looking good. I have heard of a lot of women who have to do the 24hr urine collection once and no diagnosis comes of it, just a precaution, kind of like doing the 3hr glucose test. I wouldn't worry if I were you and it sounds like you are in great hands.


    Just listen to your body and make sure to report any worrisome symptoms (I know, everything feels worrisome right now!) especially severe headaches that don't go away with rest/water/Tylenol, vision issues like tunnel vision or dark patches, extreme dizziness when changing positions from lying to sitting to standing, really significant swelling in your ankles/feet that feels doughy (this is called pitting edema), or huge changes to your face related to swelling (I looked like a completely different person...my nose was HUGE and I had no definition in my chin so I just looked like a big balloon head!). Obviously, these are pretty late signs so having one high BP reading wouldn't cause any of this, but I just say it because I ignored all these symptoms until my BP finally reached 200/180 (the midwives I was seeing had been recording my BP as "high normal" because I was morbidly obese so they didn't raise alarms when I was 140/90, 150/95 etc. Not a good situation).

  • Try not to stress about it. The 24 hour urine collection is a bit annoying but not too bad. I've done it already once. It's good for your doctor to get a baseline of where your protein is now in case your blood pressure continues to rise the rest of your pregnancy. My blood pressure has been high on several occasions at the doctor's office so I was switched to bi monthly appointments early, and measure my BP at home. I will be soon having non stress test and an ultrasound to check on how the baby is growing.

    This might be just reiterating what others have said, but I have white coat syndrome and get very anxious when I go to the doctors (been like this for 15+ years, and I have some anxiety issues) so my BP has been running high this whole pregnancy. 140/90 would be a decent number for me to get at an appointment, but I monitor at home and sometimes my BP is even borderline low! I've had no protein in my urine and I haven't had to do a 24 hour collection but you could have just had a fluke reading. If I have a little too much sugar or salt, I can feel my BP raising a little bit. BP can be affected by so many things, so try not to worry yourself yet. Plus it's very normal for BP to go a little up in the third trimester anyway, so it's possible its just gonna be a little higher right now and you'll just have to monitor it!
