New Here- Hi Everyone!

Hi Ladies!

 My name is Sam and I am new here so please forgive me for any missteps. I hope everyone had a lovely weekend.  I am looking for any kind of help/advice.
I did spend sometime lurking past discussions on this topic. 
My husband and I have been trying for about a year. I am 29 my DH 30.  I had some prior health issues, which brought me to the RE earlier than expected- ovarian cyst that caused scar tissue build up on my right ovary and in my fallopian tube. I had an HSG done in Dec. and luckily everything was cleared out. The RE suggested my husband get a semen analysis. We got the results back over the weekend and they were:
Concentration: 40mil
Motility: 33% 
Progressive Motility: 27% 
Morphology: 1.5%

This was somewhat of a blow to us because we just assumed he was fine. I was just curious if anyone had similar results and have they done anything to help improve results? My husband is healthy-no smoking no drinking. My RE recommended taking vitamins and the whole avoid briefs, laptops on laps, hot tubs run down. She said we have a few options: continue trying naturally for however long we feel like it, starting fertility drug treatment, and/or IUI.

  • Sam, hi and welcome! We have a similar story, my dx was a slightly elevated level of prolactin. But DH has very low morphology, I do not remember the exact #. We started actively trying in November 2012. We tried 4 IUIs in 2015 that were unsuccessful. Do you know what the morphology issue is? Our problem is that very few of our sperm have acrosomal caps, which is what penetrates the egg and allows the sperm to enter - big problem! If I were to do it again, I would just go straight to IVF and skip the IUIs - our odds were SO low w/ IUI. DH did try all of the recommendations and his numbers went up a little, but we still have found ourselves currently in the middle of IVF. Obviously every person's story is different and you guys have to do what you feel is right for your family, but hopefully my experience helps! Let me know if you have any other questions. 
    Married May 2009, TTC since November 2012 (Together since 2006 ish - had my eyes on him since 2001)
    Me: 32 (33 in May), Him: 37 (January)

    DX: Me: High Prolactin, Possible Autoimmune Disease Issues, though RE not concerned (?)  New RE has a plan!!
           Him: Minor Varicocele, low morphology, slightly low count

    History:

    Beta 5/9/2016 BFP!!
    Embryo transfer scheduled for April 28, 2016 and beta test May 9, 2016 (day after Mother's Day!)
    Transfer Meds include: Lupron Depot (4/1), Minivelle Patch (every 3rd day), Estradiol (3x daily), Amoxicillin, Progesterone in Oil, Methylprednisonlone. Lovenox and baby asprin added after transfer. 

    3/22/16 - Sono Saline ultrasound cyst to be aspirated on 4/1/16 if not cleared up by 3/29 US - It cleared on its own
    Retrieval 3/4/16 - 26 eggs retrieved, 23 mature, 20 fertilized, 14 embryos currently frozen
    Starting IVF Stims on +/- Feb 22, 2016
    HSG scheduled for 1-26-16 - All clear "beautiful uterus" (though inverted)

    Switched clinics and now prepping for IVF in February / March

    Fourth IUI -  CD3-7 100 Clomid w/ Ovidrel Trigger - December, 2015 - BFN
    Third IUI -  CD3-7 100 Clomid w/ Ovidrel Trigger - November, 2015 - BFN
    Second IUI - CD3-7 100 Clomid w/ Ovidrel Trigger - September, 2015 - BFN
    Started Prolactin Medication October 15 - Levels quickly regulated to with in normal range
    First IUI - CD3-7 100 Clomid w/ Ovidrel Trigger - August, 2015 - BFN
    After no BFPs (ever) First RE/Urologist visit in Feb 2015
    HSG w/ OB, 2014 = all clear
    Trying to conceive since November 2012

  • thank you for the feedback. My RE didn't specifically say what was exactly wrong with his morphology just that it was at 1.5% and they look for it to be 4% or higher. My DH and I talked about waiting another cycle or two and then having him go to a urologist who specializes in fertility issues. Good Luck with IVF!!!!

  • Hi @sammyl1221. Sorry you have to be here.

    Our RE indicated that most of the medicines out there to improve DH's results are not proven. We didn't ask about things like the laptop, etc. because we had already been doing those things. In our case (low motility and count, normal otherwise), RE said our chances with IUI were only about 5% at best, and that he recommended going straight to IVF. We're still getting some additional testing to confirm.

    One thing you might consider is another SA. RE indicated that the biggest benefit to IUI might be additional data points on DH's stats, that they could be different depending on the day and it's hard to say whether the day he got testing was a "good" day or a "bad" day for him.

    Anyways, not sure if this helps since my DH and yours have slightly different issues, but thought I'd pass on what I could.

  • Welcome @sammyl1221 ! We also had a similar experience to you. We tried two rounds of IUI with DH taking the vitamins and drinking a special tea, the works. We initially looked like good candidates for IUI because most of our trouble was motility, but after the IUI wash our numbers looked even worse and we had less than a 3% chance of conceiving with IUI or naturally, so we cancelled our cycles both times and moved to IVF. Sadly, I have not heard a lot of folks that had their DH's numbers improve drastically with any meds.

  • Thanks... my RE did say that when they did the wash(or maybe it was a different procedure?) that his motility went to 92%.
