Follicle/Monitoring Question with Meds

Hi! I searched this forum and didn't see anything directly related to what I was trying to ask - and this is really only a question for those of you that have gone through monitoring and have used medicine along the same lines as what I am using.

Today is CD 9 and I just wrapped up my Femara (Letrozole) and 2 Gonal-F shots at 37.5 iu. We went in for the u/s to see where we were in size, and I had one at 14 mm and one at 13.8 mm. My question is - is that good? Everything that I have found or read, has had those sizes at later dates in the cycle (like around day 12-13). Basically, I don't want to be stimulating them too fast if that is even a thing or possible. Also, how many eggs do you think would release? Both because of the medication or just 1? I'm waiting on a call back from the doctor to plan the next u/s visit and lab work to check levels, but just thought I'd try and get some feedback from anybody who has gone through similar or has any knowledge that can help. :) thanks y'all!

  • Hi! I've only been on gonal-f and had a higher dose. I had one at 13 and one at 12 around that time. They kept me on that dose until next u/s and then lowered it. They both stayed that small and another follicle took over (I have PCOS...so lots of follicles to choose from)
    If the follicles are both on the same side, only one releases, are they on separate sides both could release. (But small odds)
    According to textbook one follicle will take over and be the 'leader' while the others will stop.
    They still have to grow, so you will be fine :)

  • I'm presuming you're doing IUI or TI with trigger?  I've had a bunch of IUIs, and I think I saw follicles in that size range around CD 9.  Sometimes bigger, sometimes smaller.

    Don't compare yourself to others.  Women with DOR have much different responses than women with PCOS, and those with unexplained infertility can be anywhere on the spectrum.

    But at those sizes, it's not time to trigger yet, but could be time soon.  And yes, if you have two mature follicles, that's good, and that was the goal of the medication.  You will hopefully release two eggs.  Statistically, you have a good chance that one of the two will be chromosomally sound.

    Next step- reciprocal IVF, using my wife's eggs, my uterus!  
    fresh 5 day transfer (2 embryos) 4/17/17- BFP! 
    Identical twins "due" 1/2/17 (but anticipated arrival sometime December)

  • @KLake42 thank you! Yes, my wife and I are doing IUI, this is our 4th. Doc just called and said to continue the Gonal-f for the next two nights and go into the office Thurs morning for another ultrasound and bloodwork. Whether I am surging on my own or not will determine the use of the trigger, however, I have it if I am not surging by the time the follicles are the size we want. I catch myself wanting and hoping the trigger will be necessary though, since it seems it's easier to time the IUI with it.

    I was on a very similar protocol for my first (and last) IUI. I did letrozole days 3-7, and 75 IU of Follistim starting at day 6. Growth was slower for me in the beginning, but I ended up surging on day 12 (with only one follie at 19mm, and a few at 15mm which we're unsure whether they grew enough to be ready by the next morning). From my US, they were planning on having me do an extra day or two of stim, but considering my LH surge, they called me in a panic in the afternoon and told me to trigger that night and come in for IUI the next day. You definitely still have a bit of time, but exactly how much, no one can tell unless you have daily monitoring at this point.
  • I had 1 follicle in each of my cycles with Letrozole, I had follicles approx. 21mm and 20mm (I think) on CD 12 or 13 (again, an estimate, don't have my chart in front of me). I would think 13 or 14 mm would be small for much later in the cycle than you are now. 
    Me: 33, PCOS with anovulation
    DH: 36, No known issues
    TTC since 11/2014
    1000mg Metformin daily

    Oct-Dec 2015: Letrozole 5mg, Ovidrel, TI = BFN x 2
    Feb-May 2016: Letrozole 5mg, Ovidrel, TI, Prometrium = BFN x 3
    August 2016: Clomid 100mg no response, Letrozole 5mg, Ovidrel, TI, Prometrium = BFN
