Hysterosalpingogram and hysteroscopy questions! Warning: losses mentioned

I had a natural miscarriage at 7w0d in early August 15'.  I then had a d&c for a possible partial molar pregnancy after a strong heartbeat was found at 9w0d in late November.  After the d&c "something" was found in my uterus by a new obgyn.  The obgyn who performed my d&c did feel like there was swelling behind the gestational sac (hence the suspicion for partial molar).  The pregnancy was not molar.  I had a hydrosonogram in December, also showing a 1cm lesion in my uterus.  A hysteroscopy was recommended by that new obgyn.  I wasn't particularly confident in this obgyn for various reasons, so I went in for a second opinion today.  He believes if a polyp is what is causing 1cm lesion, rather than causing my miscarriages it would have prevented implantation. I'm now finding out that a hysteroscopy might not be the best course of action for me, but rather a hystersalpingogram might be better.  I've just been referred to an infertility specialist.  My out of pocket cost for a hysterosalpingogram is 50% of the procedure since my health plan through Kaiser only covers 50% of fertility treatments.  It also won't tell me about what this lesion really looks like.  We can presume that at least one of my tubes is not blocked, since I was pregnant twice (both times from the left ovary).  I know it will tell me more about the shape of my uterus, and if the other tube is blocked.  Have any of you been through this and can tell me some positive stories?  I'm having a really hard time staying positive.  It seems I try to look to hope, but every time I have a game plan something changes the plan.  Thanks in advance for your help.

  • I haven't experienced this specifically, but I have kind of spent time with my OBGYN who seemed to be just throwing treatments at me that were not working. She just recently referred me to a fertility specialist/RE and for the first time in a while, I feel like we may get some answers. So, that is one positive part of your situation. You may finally end up where you will get the answers you need. Good Luck! FX'ed for you!

    Jan. 2016: 1st round of Clomid - stopped due to OHSS

    Feb 2016: 1st RE apt - diagnosed with PCOS still possible uterine septum. Put on BC for cysts, HSG will be in March

    March 2016: HSG showed both tubes blocked (right tube holding fluid), no uterine septum, need surgery

    April 2016:  Surgery to remove right (hydrosalpinx) fallopian tube and adhesions from the left - all went well!

    May 2016: Waiting to start 3 months of un-monitored cycles in July with metformin and letrozole. If no success, going straight to IVF!

  • I haven't experienced this personally either. It could be many different things though, so I imagine that further testing will be needed. The RE is a better person to go to about this kind of thing, so I think you're in good hands! Keep us posted!

    1st beta 2/1 = 110. 2nd beta 2/3 = 332.

    First ultrasound 2/19 - one baby with a heart rate of 127! EDD 10/11/2016

  • Sorry you're going through all this...I had a hysteroscopy back in July for a suspected t-shape uterus (it was subtle on the sonohysterogram I guess), and it was thought that this would explain why my lining wasn't getting thick enough. I had never seen a positive before (but keep in mind I was then also diagnosed with severe endometriosis). For me, the hysteroscopy turned out to be the best thing as he could see the shape of my uterus from the inside. Because from the 'entrance' he couldn't see the opening of the tubes, he concluded to a t-shape uterus and corrected that right then and there. I saw a difference at my following cycle (much heavier, followed by a nice lining), and thankfully we got a BFP the first cycle after that (meds and IUI, just to be more aggressive).

    My RE specializes in minimally invasive gynecological surgery (MIGS), and followed the hysteroscopy by a transvaginal endoscopy to see outside the uterus and make sure all was fine there too. That's when he discovered the endo. My recommendation would be to find an RE that specializes in these types of delicate surgeries/interventions and see what they say. I'm not convinced that getting your tubes checked would be the highest priority considering two pregnancies implanted (i.e. They met in the tube, and made their way to the uterus...it's once there that issues happened if I understand correctly). I hope they find a solution for you!
  • I'm so sorry you are dealing with this and feel like you may be getting the run around! I don't have much experience with all those tests but do have Kaiser as well.

    I know they have a lot of different plans) but mine also covers 50% fertility. I want to say when I had my HSG done as prescribed/recommended by my Obgyn, it wasn't that much bc technically and was done through Kaiser not a fertility specialist. I don't recall them saying much about the shape of my uterus and mainly used it to make sure neither tube was blocked. It was uncomfortable but not too painful and over fairly quick (maybe 30 min from undressing to dressing) Could you have your Obgyn put it in the system rather than the RE if cost is the deciding factor?
  • I've had an HSG and an HSS ultrasound. Everything looked fine during my HSG in August 2014. When I did the HSS U/s in September 2015 they found a polyp and did surgery to remove it. I too have Kaiser, but because I have an individual plan in California they cover nothing for infertility treatments, which means I pay for everything out of pocket at a specialist. I don't really have a positive story as I have yet to achieve a successful pregnancy. I hope all goes well for you!

  • I have a similar situation...I think.
    DH and I had been ttc for a little less than a year and my obgyn did a few tests including sending me for an HSG. Everything looked fine according to her so she suggested a fertility specialist and I did my own research as to where I wanted to go.
    My current dr is a fertility specialist and on my first (3 hour) visit we discussed some stuff and he did an exam in which he did a vaginal ultrasound. He found what he thought was a small septum in my uterus and suspected I may have endometriosis and maybe pcos.
    He did an HSS and found exactly what he had expected. He was able to remove the septum and I went on Lupron to clear the endo up. He then put me on some BC to get my hormones up from the Lupron. I got positive tests the first 2 months but they ended up being chemicals. Now dr has put me on clomid and I'm waiting to see if it worked.

    I feel like my dr had a plan and up to this point everything has been billed as a medical need because it has to do with my overall health and may help with a pregnancy. I have reached the point that we will have to pay out of pocket 100% though, totally stinks!

    I think you need to decide if you're comfortable with your dr and if not you should look for another. I didn't use my obgyn's recommendation. I dont think my new dr is an RE, not sure exactly what the difference is...he's and obgyn and specializes in fertility and balancing hormones. I wish you the best of luck!
  • Also, we have been working with our fertility dr for 14 months. Ttc for about 2 years, 2 months.
