Infertility

Intro and IUI question

Hello everyone,

I'm Jenni, been on the ttc over 35 boards for a while now, and moving here because we start IUI next month due to unexplained infertility, possible low sperm count (12m)

Also, I was wondering if anyone can answer a question I have a question about IUI: I'm going to a clinic with a great reputation, but something I can't understand is that they trigger 24 hours prior to the IUI. Is this standard practice with other clinics too? I have asked them the question and they say there's no data supporting that this is less effective, but I've read that 36 hours prior is more ideal. I know the trigger shot causes you to ovulate 36-40 hours later. I feel a little run over when I ask questions - it's basically always "theres's no data supporting one works better than the other." Ugh. I guess I'm just freaking out about wasting money since we have to pay out of pocket for all of this.

Anyway, hi again and thanks for any help in understanding this whole thing.

Re: Intro and IUI question

  • Hi and welcome! At your clinic do you only do one IUI? When I was doing IUI I always went in two days in a row to do the actual IUI, so trigger one day, IUI the following two days. I think their reasoning is probably that sperm survive for several days after the IUI and they need to be in your body before you ovulate to have the best chance of fertilization.

  • Welcome I only get one iui a month and it's usually 36 hours. But it seems like every office is different maybe your follicles were big enough that it looked like you would ovulate sooner or maybe there was something in the bloodwork with your levels. I would just ask more questions to the dr next time. Don't stress yourself out though you still can BD the days after
  • I did 3 IUIs and all ended up being trigger between 7 and 10 pm one night and then IUI 2 mornings later, so about 36 hours after trigger.  However, my doctor always decided based on my bw when to schedule everything, so the sono before I triggered, usually it would be "expect to trigger tonight, and then IUI tomorrow or 2 days from now, we'll let you know based on bloodwork."  My RE also did one IUI per cycle, and then the nurses suggested we BD the night of the trigger and the night after the IUI (taking a break on the in between day).

Hm I didn't realize this was the norm! I did my trigger shot around 10 am Friday and then got the IUI around 2 pm Saturday so a little more than 24 hours later.

  • My IUIs have been between 24 and 36 hours post trigger. From my understanding, it's better to be early than miss ovulation so maybe that's why they want you at 24 hours?

  • Hello and welcome!  It seems like both 24 hours and 36 hours post trigger are normal, and if your RE says the research doesn't show that one is more effective than the other, I would take their word for it.  When I did my IUI it was done 36 hours post trigger and I could feel myself actively ovulating while I was at the clinic for the procedure.  I've never had cramping like that before.  Since sperm can survive in your tubes for a couple days, having your IUI before you actually ovulate should be just fine.

  • ok thanks for all your help! Since I know it generally takes my body a little while to ovulate, I actually did find a new clinic that performs IUI depending on bloodwork and sono, and also can do 2 IUI's rather than the cookie-cutter single IUI 24 hours after trigger because I did find some research that differed from what my doctor said. I don't think I would've had the guts to call around if I didn't hear from you all that the 24 hour thing isn't your only option
