Maternity Leave Form on EDD

My OB told me that I need to fill out a form on edd.gov and mail the first week I take my leave to start getting checks... I looked... there are a few forms and have no clue which one to fill out. I am so confused on what to do. I still have a few weeks to figure it out but just in case I want to know ahead of time. My work doesn't seem to have a clue. Help! Any of you girls know what to do?

  • Your work doesn't have an HR dept?  They are the ones who take and process the paperwork anyway. . .  And you only get money if they have some sort of short term dissability plan  . . .  Mine is through Prudential, I sent all my paperwork to my home office and I need to contact Prudential in order to get my 60% of pay . . .

  • I know crazy right? I work for a smaller company and we don't really have a definied "HR" department... I guess it's different cuz I'm in CA. I for some reason thought it was the same for everyone... I feel like an idiot... I'm going to just keep asking around.. UGH!!!!

  • I'm in CA. When I was PG with DS, I went to a local EDD office and just filled everything out there. Can you try doing that?
  • I'm in CA too.  My dr had the EDD paperwork that I filled out, then returned to them for them to fill out the rest.  Then they mailed it in.

  • do you have an HR dept?  i was lucky my boss went through all of this already.  Dr faxed in Short Term dis stuff, i just have to call and say "i delivered, and its a c-section" they confirm w/ Dr.

    as for FMLA, i have apparently one sheet of paper to turn in and i'm done.  *shrugs*

    Call your HR dept if anything to see what they need, and if going on STD, call that company to get anything you need help with.
