TTC #2 w/ PCOS - advice/experience wanted

DH and I have decided to TTC baby #2. I have PCOS but conceived #1 w/ 100mg of clomid and an hcg shot at the first round with an RE but after trying for 2 years before getting to an RE.

I took clomid CD 3-7, 100mg. Only 1 follicle and it grew 1.2 mm with the clomid (ended up being only 13mm total). She immediately put me on 150 MG clomid without waiting out my cycle. She said they were basically just going to "pretend" I'm back on CD 3 and take 150mg for 5 days and then come check on pretend CD 12 for a follicle check. If it doesn't work this round she said they will try something "more aggressive" but didn't detail what that meant.

I'm on CD7 and have been having massive headaches and uterine cramping.

Any experience with this immediate nice from 100 to 150mg of clomid? Are the symptoms normal? I didn't have them when I was TTC baby #1 on 100mg clomid. Any positive experiences with the higher dose? If 150 MG doesn't work, anyone have experience with the next step?

  • I don't have experience with doing a second round of clomid during the same cycle, but 50 mg and 100 mg clomid gave me really bad headaches each month. I talked to the PA handling my case at my fertility clinic and she mentioned that due to my neurological side effects, we could consider femara next month. I've been researching it a bit because it doesn't make sense that it would be lesser or different side effects since it practically does the same thing as clomid - blocking estrogen receptors. It's worth talking to them about alternative meds for the next cycle (should this not be the last one) if the headaches are bad. Tylenol only does so much. good luck

  • With my first, I did 2 clomid IUI rounds, and then we got "more aggressive" and added bravelle.  It is called a "hybrid" cycle and they have success rates close to that of all injectable cycles.  We did 3 hybrid cycle IUI's.  After that, we did an all follitism IUI and conceived my son. I have had headaches with high IVF meds, but not really anything else. Caffeine really helped my headaches go away.  I am a bit surprised your RE isn't giving the 12mm more time to grow.  Usually when follicles reach 12mm they continue to grow. What day were you checked and it was at 13mm?  I was always a late grower on clomid, and even injectable cycles too.  I never triggered before CD18 for any cycle.  Even my first IVF was a slow starter.

    IVF #1 9/15 Antagonist- 19 R 1F (Frozen day 1 @ 2PN stage) Diagnosed with SER aggregates in 13/19 eggs after failed fertilization
    IVF #2 11/15 Antagonist w/ICSI  (Higher stims & Letrosole added last minute on stim day 7)- 39R, 24M (20/24 SER free), 15F (14/15 SER free & all frozen @ 2PN stage) 
    FET#1 Cultured all 16- 2pn embryos to day 5- transferred 1 good hatching blast and 1 early blast on 1/28- None to freeze - beta 2/5

  • When they checked I was at CD 17. Guess they didn't want to proceed. 150mg of clomid seems to have much less serious effects than 100mg. Still have them, may not as severe.

  • Well I don't have experience just starting a new cycle like that, but I do have experience with Femara (similar to clomid) working for my first and now not working. So, my doctor added injectibles (Follistim). It is still not working, but I think that is often the next step- adding more drugs. 
  • Went in today for a checkup. I have 2 follicles. One is 16mm, one is 14mm. Going to go in again on Monday to see if anything has developed further. Was kind of bummed out to hear that, especially with the higher dose of clomid. Hoping for a better day on Monday!
