You want to know why he's sleeping bad? Because he's a baby! For real. It's as simple as that.
New parents or first time parents get used to a certain sleep pattern and then when it changes (like it always does), they try to find a reason for it but there isn't a reason other than basic, normal sleep regression and it will keep happening over and over.
You aren't doing anything wrong. Nothing is wrong with your baby...you just have to wait for it to pass. Sleep gets really challenging around the 4 month mark; google "4 month wakeful" and you'll see. Also around this age naps will get shorter, usually 30-45 minutes and they take quite a few of them. Also normal.
At this age you really should try to wean him off the swaddle. He's getting a little too old for it and might make sleep worse because he's fighting to get out.
Hang in there, I know it sucks but it will get better and a little more consistent in time.

Dude, I totally know because I did the same thing with my first kid. I figured if I could figure out what the problem was, I could fix it and we would ALL get more sleep. Sadly that wasn't the case.
I've been so much more relaxed with kid #2 because I know what to expect. I promise it will pass and around the 5-6 month mark you can start doing some sleep training if you want. That will help tremendously.
