How late is it possible to ovulate after Clomid?

Hi, DH and I have been trying for 2 years. I have PCOS and am overweight (5'6 - 196lbs.) Cycles have been coming on their own for a year varying from 26-29 day cycles. Started out weighing 213lb in August. Am trying to lose more to help my ovaries out and be healthier. Currently on 1500mg Metformin & 50mg Clomid (started Nov). Started Metformin 2 months before Clomid. I have been on Clomid 50mg once before and never noticed if I O'd, but that was over a year ago and with a different doc. New OBGYN suggested going through 4 Clomid cycles back to back since it could take more than one month to get my ovaries in gear. I ask doc if she could monitor Clomid Cycles with U/S and was told she would send me to an RE after the 4 Clomid cycles and that she doesn't do that. I have never in past cycles got a + OPK. Then again I stopped testing around CD20. This time I took Clomid CD5-9 and started testing CD11. As of Nov 16th I am on CD 15 and still no + sign of O. Just wondering if I could either be ovulating late or my LH levels are too low to result into a + OPK. DH and I are BDing every other day to try to catch it just in case levels are low. I'm trying not to worry and telling myself that we are doing everything we can right now, and in words of my OBGYN: "I know you wanted to be pregnant like yesterday, but these things take time."   • @lovelacep welcome and I'm sorry that you are here. I too have PCOS. Because of the hormonal imbalances that are caused by PCOS it is nearly impossible to test for the LH surge with an OPK monitor since it can lead to false positives. Also it is highly recommended to be under the care of an RE who will monitor your cycles when taking Clomid because it thins your lining and i believe there is a lifetime maximum when using Clomid. Please try and find an RE to help you. I understand your OB thinks you should do 4 rounds of Clomid without monitoring but that may be excessive and i would recommend you to be safe than sorry and get a second opinion.   • I would really recommend asking for a referral to a RE now. You will not be having effective clomid cycles without proper monitoring. My doctor said not to even use the home ovulation monitoring while I was on clomid as the medication impacted the results. Further, how would you even know if you are responding to the clomid? I did 6 clomid cycles, monitored by ultrasounds and at varying dose levels. I didn't respond. I would hate to see you wasting your time which, without monitoring, you may be doing...

    Thank you @vnegron87 for responding! When I had my blood work done I was told that my hormone levels all looked fine and nothing out of the ordinary. They ruled out thyroid and other problems before diagnosing me. I guess since I had no period for 6-8 months about 3 years ago and that my ovaries have the string of pearls is how they diagnosed it. As far as seeing an RE, do you think it would be okay to go 2 cycles of Clomid unmonitored with my OBGYN then seek for an RE? Just wondering since my ins does not cover RE or any infertility help. Thanks for your advice and will take it into great consideration :)

  • Btw, I'm sorry i forgot to answer your initially question. I did a double cycle of clomid this month, 50mg CD3-7 and 100mg CD15-19 because my follicles didn't develop on 50mg (I'm only able to know that because my RE was monitoring their growth). The second dose boosted a follie to 18mm and 36 hours later i triggered with Ovidrel. I then Od 36 hours after that and had 2 back to back IUIs (currently in TWW). That puts my ovulation day this cycle at CD 22. Many factors can effect when you ovulate, and even though you've had relatively shorter cycles (assuming the Metformin has been helping) factors such as stress or sickness can delay ovulation in any given cycle.

  • @lovelacep thank you. It is difficult to go through this, even more so when you have to pay out of pocket, i can understand why you are trying with the OB as long as possible. Has DH had SA? You should do whats best for the both of you always :-D

  • @vnegron87 No he hasn't had one because he doesn't have health ins. I know it doesn't cost a ridiculous amount of money but we have discussed that that is the next step if this doesn't work. We have been kind of hoping we would get the BFP soon so he doesn't have to get it done. The good part is that he is all for it and trying to help me with everything.

  • An SA is 50 bucks.  Please get an SA for him and an HSG for you.  Your tubes could be blocked and all the Clomid in the world won't help get you pregnant.  Similarly, he could have sperm issues preventing pregnancy.  Unmonitored Clomid can cause permanent damage to your fertility.  Please get to an RE.  Their insurance people should able to review your coverage to see what is covered.
