is it stress, excitement, pain or something else that is making it difficult to sleep? I've had a cold so I've been waking up fairly regularly around 3:30-5 which is crazy early for me but once I'm up I don't go back to sleep.
I'm guessing that you've already tried the normal remedies for pain? pillows between the knees, under the belly? I find that sleeping at more of an incline helps because I can distribute my weight a bit more towards the back when I'm not completely flat. Do you have a recliner or couch that you can move to? Either that or maybe just a Tylenol would help- I'd ask the dr about that though.
For the anxiety/busy mind try a sound app. I have a free one called relax melodies by ipnosoft and you can customize them and I really enjoy it. I used to have a very high stress job and it was almost nightly that I couldn't turn off my brain but having the sounds to focus on really helped. You can make them as complex or simple as you want.
Are you using your phone, computer, or watching tv right before bed, or at night when you wake up? I've found that if I do check mine, I have a hard time going back to sleep. I think it has something to do with the brightness of the screen and tricking your brain. Maybe try reading a little, with a dim light? This puts a slight strain on my eyes but makes them tired- usually always does the trick for me and I fall asleep soon after beginning to read. This is also why I can never finish a book...
I have random insomniac nights interspersed with nights where I'm out cold (without bathroom wake ups, even!!). The thing that typically makes the difference is when I can convince DH to include an arm massage with my short nightly back rub. There is something about an arm massage, starting at the shoulder and going all the way to the fingers, that relaxes the whole body in a way that back/shoulder massages don't. Try it! And sometimes I add benadryl to the mix.

I've struggled with insomnia forever and this is a huge part of it for me. The light on your cell phone/iPad screen is blue light and is activating. They have blue light filters in the App Store that claim to reduce this, which I haven't tried but have been recommended to me! May be worth a shot if you tend to use your phone or tablet before trying to sleep. I just try to avoid it altogether now, which seems to work out pretty well.
So I got the OK from doc for 1 unisome gel. I was out cold. Best sleep in a long time! Am a bit tired still, but totally worth it. I also would only use these once in a blue moon if I really need it, bc it is strong.
