New to the Bump....Looking for Insight

Hi all! I am new to the bump, but have been unsuccessfully TTC for the last 18 months. I am sorry about the long post, but I do want to give some back story. I am 28y/o healthy female, DH is a healthy 33y/o and we have been married for 3 years. I do have an issue and was wondering if anyone had any insight about it as I have seen 2 OBGYNs and a RE without any clear explanation.

I was wondering if anyone else has experienced recurrent heavy mid cycle bleeds like I have. Basically, my period lasts between 8-10 days and is on the moderate/heavy side. The bleeding will stop, I will get a change in CM, and an LH surge between CD12-14, and then one day after surge, the bleeding will begin. It ranges from light spotting to moderate bleeding requiring tampons. It will last until CD 29 ish and then the bleeding changes, becomes heavier and more consistent through the day and I will develop AF like symptoms, cramps/bloating ect, AKA turns into a period. It has been consistently like this every month. Due to the very cyclic nature of the bleeding, and that I get a “true” period every month, the GYN and RE feel I am ovulating.

I was prescribed progesterone suppositories by my GYN, for a “luteal phase defect,” and this will stop the bleeding about 2 days after starting them, however I don’t understand why I am bleeding in the first place, and no one can seem to explain it. Also, I did take a round of clomid this last cycle and I did not experience ANY bleeding at all, however I also did not conceive.

So far, I have undergone a saline sonogram, HSG, MRI, multiple trans vag U/S, and all hormone testing. The only abnormality in anything is that there was a hemorrhagic cyst found on my right ovary. It seemed persistent, and the question of an endometrioma was brought up, however the RE does not believe it is, and does not think it is worth undergoing a laparoscopy. Also, my husband was diagnosed with “mild male factor,” he has a normal count, but low end normal motility and low morphology.

IUI has been recommended at this point, and the RE is very optimistic that it is all we will need, however without understanding the underlying issue, I find it difficult to move forward with such expensive therapy (or believe him  for that matter.) Our insurance does not pay for any fertility medications or procedures, and I am reluctant to spend thousands on a procedure that may not work. Also, I think I am scared that if IUI fails, we financially could never afford IVF.

Has anyone experienced bleeding like this, and what was the reason, and were you ultimately able to conceive? I appreciate any insight, and wish everyone the best of luck on your TTC journey.

  • It sounds like you may have PCOS. My cycles were the same and I was diagnosed with it.

  • Have you had your progesterone tested on day 21 (or around that?). I have been diagnosed with endometriosis and used to have spotting for a few days before AF, but nothing like what you're describing. I did have a laparoscopy, and we did Femara, Follistim and IUI the first cycle after and this was succesful for us. Unfortunately, the symptoms experienced often don't correlate with the extent of the endometriosis, so unless they do a lap, they won't know whether that's what's preventing you from conceiving. I had my right ovary completely stuck in endo, and so any month I ovulated on that side, there was no way for the egg to make it to my uterus. My RE seems to have the exact opposite philosophy from yours. He believes in minimally invasive surgery (he actually did a hysteroscopy and transvaginal endoscopy before going for the lap...during the endoscopy he saw how bad things were, so he switched to lap) in order to reduce the rate of having to go to more complex and expensive treatments like IVF. May be worth getting a second opinion?
  • You must find the cause of all this bleeding. Not to scare you but it could be anything and I would hope it isn't some kind of cancer. Could be endometriosis too. most definitely you should get a second opinion. ALWAYS follow your gut instinct. .. if you feel it's not right, it probably isnt.
  • I would absolutely get a second opinion.  As @KidShrink said, doing a minimally invasive procedure may clear up what's going on, rather than moving on to fertility treatments that may not work due to whatever is causing this.

Harmony: negative; level 2: babies look great and are boy/girl! :)
  • Second opinion. If you feel it isn't right, go with your feeling. I have not had what you describe (I don't think), but I have endo and it took at least half a dozen doctors before I was properly diagnosed. Everyone just kept saying I had bad periods and it was normal. It's not normal. I hope it is nothing too serious in your case. Best of luck and welcome!
