Smooth Endoplasmic Reticulum Aggrigates (SER aggregates, SERa)

I am posing an update on my cycle so that other people down the road searching will have another case to look at.  So far I have only found two people online, both on TB, that have had this disorder, as it is VERY rare. 

I had my IVF in early September.  They retrieved 19 eggs and we had only 1 fertilize.  I just had my WTF meeting and was diagnosed with SER aggregates (Smooth endoplasmic reticulum aggregates).  Apparently this is very rare and my doctor has only seen this once before.  We had major issues with IUI years back and no one could figure out why my eggs weren't fertilizing, and now it seems we have an answer.  We will be moving to IVF #2 and I am starting provera for it Sunday.  The stim meds will start November 2nd.  We will be doing a much higher dose of meds and hoping to shorten the stims to reduce the affected eggs.  Out of the 19 eggs, 13 of them had this disorder and 6 were normal. The one that did fertilize was not affected. There are studies showing that even normal eggs from SERa+ cycles can have issues.  We will be doing ICSI on all eggs next time and hope to only use the normal eggs.  My doctor was open to using some of the SERa+ eggs, but this has not really be studied much and there have been reports of malformations and other disorders from using these eggs, but I could only find one study that actually followed the use of these eggs, and I believe most were born just fine.  It is very rare and there aren't many studies done on it.  The most indepth study had 183 pregnancies fro SER+ cycles and out of those, 171 were normal healthy babies.  That is for positive CYCLES though, not the eggs themselves.  I believe out of those 183 cycle, only 22 used affected eggs.  Apparently high e2 can cause eggs to have this disorder, but my cycle had VERY low E2 and it never got above 1000, even after trigger, so it does not seem to be my issue. My E2 was 736 at trigger with 18 eggs measuring 15mm and above.  The day after trigger it was 992.  We shall see.........

IVF #1 9/15 Antagonist- 19 R 1F (Frozen day 1 @ 2PN stage) Diagnosed with SER aggregates in 13/19 eggs after failed fertilization
IVF #2 11/15 Antagonist w/ICSI  (Higher stims & Letrosole added last minute on stim day 7)- 39R, 24M (20/24 SER free), 15F (14/15 SER free & all frozen @ 2PN stage) 
FET#1 Cultured all 16- 2pn embryos to day 5- transferred 1 good hatching blast and 1 early blast on 1/28- None to freeze - beta 2/5

  • wow. i've never heard of this. ever. are there support systems for you? like, is there a SERa support group where you can "meet" others who have this?  i surely hope so. i can imagine how defeated/frustrated/alone you must feel, but i am glad your doc has a new plan to try his best to make it happen.  please keep us updated, i'll be keeping you in my thoughts.

    IVF #2.1 March 2015 cancelled due to dominant follicle.

    BFP #6  (SUPRISE!) 3/19/15.  EDD 11/30/15...  CP at 4w2d.
    IUI #2:    Clomid + Follistim = 3 follies. BFN.
    IVF #2.2 May 2015 - horrible response to micro lupron flare protocol: 3R, 3M, 3F. 3dt of 2 Grade 3 embies. BFFN. 

    BFP #7 (beyond surprised again!) 4/26/16. EDD 1/5/17. beautiful betas!!!! and then near-fatal hemorrhagic corpus luteum. turns out baby was ectopic after all; another lap 5/6 (@24dpo).

  • @jodee37   I found two people on here throughout TB history that have had it, and I sent them PM to get as much info as I can, but other than that, I haven't found one other person online that has it, let alone a support group. One of the gals looks like she got pregnant on IUI after doing many IVF's (I am guessing she switched to IUI due to the high estrogen with IVF as being known to cause this problem).  My problem was low estrogen if anything, not high.  The other gal looks like she got pregnant with her second IVF and the aggrogates were gone. I wish there were more than 5 studies too!! Some of the studies seem uplifting and positive, where others are horrible and scary as h*ll. We shall see what happens :)  It is really out of my hands.  I just wish I had more people and cases to look at for information, so I posed this here so someone in the future can have 3 people to check out :)
