No sac at ultrasound? Is it normal?

Good morning ladies,

I've been stressing since Monday after having an ultrasound and nothing showing up except for a cyst. I don't know how many weeks I am because my doctor didn't tell me. My last period was on August 8th so I thought I was 5 weeks and a few days.

When I went into my appointment all they did was take my height and weight. No urine test or blood test to confirm that I was pregnant. I literally was placed in a room, the doctor came in and asked five questions and then gave me an ultrasound. He told me there was no sac and that I need to watch out for signs of an ectopic pregnancy. He was very quick and I was in and out of there in less than eight minutes.

I'm changing doctors and have another appointment on Monday but I'm stressing out.

  • I'm not sure what to say other than you are right about changing Doctors. That sounds very strange that you were not offered a urine or blood test.

  • Yes it was very strange. I'm hoping my new doctor will be more thorough.
  • I am surprised that they would see you that early for an ultrasound. There is not much to see at 5 weeks. My OB will not see you before 8 weeks. The heartbeat doesn't start until closer to 6 weeks.

  • I honestly didn't think they would do an ultrasound either. When I made the appointment I made it to confirm that I was pregnant but they did none of the sort. 
  • Good luck on your next appointment with a different doc and I hope they can give you a peace of mind than what this doc did. To not take a urine or blood test and go straight to an ultrasound so early with no explanation is not customary. 5 weeks is so too early you're not likely to see anything.

  • Thank you! This new doctor won't be giving me an ultrasound until I'm 12 weeks. With my first pregnancy I didn't get an ultrasound until I was 10 weeks. I'm hoping it is just too early to see anything and praying for the best!

    Seems odd that they wouldn't have given you a urine test to confirm if that is why you made the appointment. As far as the ultrasound even if you are 5 weeks (which you might very well be a little earlier depending on when you ovulated) then it is normal to not see anything on the ultrasound. Good luck with your new doctor!

    This happened to me too.. Few days after we found out that my husband and I are pregnant, we went to our very first "emergency" OB appointment because I was having a bad lower abdomen and hip pain. I had an trans-vaginal ultrasound at the clinic and they couldn't find the sac (they suspected an ectopic pregnancy). So my OB sent me to the hospital to have my blood drawn and another trans- vaginal ultrasound there. They couldn't find a sac neither. Had another blood drawn after two days to check if my hormone level doubled. Unfortunately it DID NOT. ON-call Ob checked my cervix and found a tissue going it's way out of my cervix and she said I might be having a miscarriage. She sent it to the pathologist to make sure if it's just a polyps or if it's the baby. Me and my husband have to wait for a couple of days for the result. That's the longest 2 days of our life that we have to deal. So after 2 days I had another blood draw to check if my hormone level doubled or decreased because of the "suspected miscarriage". FORTUNATELY it doubled from 900 it went up to 2500 :) had another trans-vaginal ultrasound and MIRACLE!! they found a sac and found our tiny bean in my uterus (so it's not an ectopic pregnancy). I am now 6 weeks and 6 days already heard our little baby's heartbeat at 6 weeks. We're excited for next week for our next appointment to hear the heartbeat again! :) Stay positive my dear and don't lose hope!!!

    So glad that everything worked out for the better @leonaps. I ended up going to the emergency room this afternoon due to stabbing pains in my abdomen that persisted. They took my urine and did several different blood tests. They sent me to have an ultrasound and a trans-vaginal ultrasound. 

    Long story short they found a small yolk sac! There was no baby to see because they said I was too early :) But they also found 3 cysts on my ovaries, the first measuring at 3 cm, the second 5 cm, and the third at 8 cm :( 
    And to top it off, I found out that I have a bicornuate uterus (it means my uterus is split and shaped like a heart :/)

    I'm so upset that my previous doctor didn't catch the two other cysts! I have to schedule an appointment to get them checked out just to make sure everything is ok.
  • @Sluiggi sorry about the cysts and your crappy doctor but congrats on the good news regarding baby!!

  • They should have just done a quant for sure. There's nothing to see on an ultrasound until your hcg is 2000 or more usually
  • You are absolutely doing the right thing by changing doctors. The very first ob I saw did the exact same thing to me at right around 5 weeks (may have been a day or two earlier). No blood, no urine, straight to ultrasound. It was insane. To add to the mess, there was a mixup with the ultrasound record, and we also ended up with an ectopic pregnancy scare. After a weekend of blood tests and an additional transvaginal, everything was found to be just fine. I immediately changed doctors! 

    Hoping you find a good new doctor. Hang in there!

  • @knitknit These doctors nowadays need to start learning bedside manners because a lot of them (excuse my language) don't give a crap about us. I'm glad everything turned out well for you and your little one, I hope your new doctor is a giant step up from your previous one.


  • Same thing happened to me. My period ended August 6. I went to my first appt Tuesday and they did a urine sample which came out positive but they found nothing in my uterus or tubes etc. I'm going back this Tuesday and hopefully she sees something so she can rule out ectopic pregnancy. Keep me in your prayers ladies!!

    Please explain ... I've never worked for a dr that doesn't care in the 6 years I have been a nurse .

    @jonesl12 Well, if you read my situation my doctor didn't. And I've had previous doctors that were very rude and I felt like I was bothering them with my presence. It's nice you work for doctors that care, but I've dealt with more than one who did not have good bedside manners. Also, being a Cna allowed me to see how hasty they are.
