Advice need? pills/injectables or all injectables with DOR diagnosis

Here is the back story..I was diagnosed with DOR(diminished ovarian reserve)  in April/May of this year.  Since then I have done 2 IUI cycles both with letrozole, the first with Gonal F and the second with Menopur.  I did slightly better response-wise the first time but the Dr said it's either just normal month to month variation or it could be the doses/meds that work better for me.  Both had 1 mature follicle the first had a possible 2nd that was 13mm at trigger and could have hit 15 my ovulation. Both were BFN

So we are getting ready to start cycle #3. When I spoke to the Dr on friday she was all for us doing all injectables this month assuming we were ok with the prices.  Our insurance covers everytihng but meds so that we pay out of pocket.

I went in this morning for my CD2 baseline and she told me that the increase in my odds going from the letrozole/injectables to all injectables is anywhere between 1-5%, so not a huge increase and that if we are thinking about doing IVF in the future it may make more sense to stick with the letrozole/Gonal F combo and save teh money for a possible IVF.  She also said if we do all injectables and see we get 3 plus follicles it's a good sign of how we might respond to higher does meds for a future IVF as well (one of our biggest issues iwth IVF is is it worth all the money for possibly a poor response and very few eggs)

 

What would you do?? The money isn't a huge issue..we would rather not spends tons unnecessarily but we cna swing it.

  • The thing about all injectibles is that you start on CD 3. From what I have read, if you do a combo Letrozole/injectible, you start the injectible on day 6 and the lead follies are already chosen by your body and the injectibles won't necessary produce more.
    For me personally, I would (and did this cycle) choose injectibles only. I am 37 and want to up my chances as quickly as possible.

  • That makes sense..I think if we do the combo again we will just get 1 follicle..i know it only takes one but why not up our chances..My husband said we can do whatever but his gut is saying stick with the combo (honestly I think his gut is related to the cost and he prefers the cheaper) I don't want to waste more time either as they say my issues are all about time running out and I know eventually if IUI's don't work I will try IVF or i'll regret it but my husband also will not agree to IVF anytime right now.  (which for someone so concerned about money all the time he would rather us put more and more smaller amounts of money out each month and then the big bucks on IVF if none work than just go ahead now, but whatever..if IUI works out then he is right I guess..

    Sorry, I am just so torn on what to do for this month..I guess when the nurse calls tomorrow morning I will see what she thinks and just make a game time decision
  • My RE gave me a much hugher success rate with injectibles. They upped pregnancy rate to 20%. Also, injecitbles change the quality of eggs. I normally have 3-4 follies on oral meds. This past cycle I had 4-5 follies with injectibles. The nurse was very concerned about continuing the cycle because injectibles typically produce stronger/healthier/better eggs than just oral meds. It seemed like I produced about the same number, but the nurse was concerned since they were Follistim follicles.
