Help! STM but different breast feeding experience

my first never latched well. I was in hospital through most of day 3 since she was born late at night. They had me pumpin hospital. She latched a handful of times but the pump felt like it drained me.
This time baby latched well in hospital and was having good stools wet diapers. Got home she started having bad latch. I think I either have clogged ducts or I am engorged. Tried everything to fix it. At first pumping and baby made breaststroke feel softer. Now not. I called baby's dr since no poop in over 24 hrs and only 3 small wet diapers. When I do pump I get about 15 or slightly more mls. She said she thought baby is going to turn corner and is calling back soon. But I feel worse. Is this normal? Should I call my dr now while I wait or tell her things got worse when I call?
