low progesterone after first round of clomid

I started my first month of Clomid last month, 50mg. On day 13 I went in for ultrasound, had about 8 total follies, one was 21mm, the rest were 13 or less. My OB didn't feel comfortable giving me a HCG shot because I had so many follicles. I used OPKs starting from CD 9 twice a day until CD 21, never got a positive. I didn't even get a faint test line on ANY of them (I was using two different brand OPKs also) DH and I BD'd about every day. I went in for my day 21 progesterone test, and it was .6 :( So disheartening. My OB thinks its a fluke ( not sure how a blood test can be a fluke?!) and wants me to stay at 50mg again to see if anything changes.  I really wanted the HCG shot but my OB said that that the shot won't help me ovulate. It only helps grow the follicles quicker. From the things I've read on here that seems like conflicting information.

Has anyone went through something similar to this? If so, what was the outcome?

  • Hi! Sorry you have to go through such conflicting info. It's a bit odd to me as all the follies under 13 would have been immature (they need to be 18-22)...you progesterone at this point could be low simply because you didn't ovulate yet (which seems to be what your OPKs are saying). I'm not an RE, but this sure sounds odd. Does he know your OPKs have been negative? If so, what does he say about that. If he also thinks that's a fluke, I would be questionning his competency...If I were you, I'd make the switch from OB to RE ASAP.

  • I agree with @KidShrink completely. I can't imagine any concern a trigger would release any of your follicles under 13. Also, as far as I'm aware, the doctors give you the trigger to make you ovulate, so I'm having doubts about the competency of your OB for infertility purposes. I'm glad your OB is at least monitoring you (most don't), but experiences like this are why the majority of people going through infertility struggles recommend seeing an RE. I recommend asking for an RE referral and seeing an RE for your next round. Good luck and I'm sorry you even have to be here!!

    Clomid 50 mg + metformin 4/15 (unmonitored) - BFN 
    First RE appt. 5/15; Natural cycle 5/15 monitored with 2 mature follicles and Pregnyl Trigger (full dose) + prometrium - BFN
    6/15 HSG - clear tubes & normal uterus; great PCT test results
    TI - 100 mg Clomid + prometrium (AM & PM) + vaginal estrace (AM & PM) 6-7/15 (monitored) --> no additional response and thinned lining - BFN
    TI - Injectables (follistim + Gonal-F, Ganirelix, & 1/2 dose Pregnyl) + prometrium (AM & PM) 9/15 --> 3-7 mature follicles (3 definites and 4+ that could have matured due to trigger) @ O -->BFN + 5 large cysts
    BC for 2 weeks due to cysts
    TI - Injectables (Gonal-F, Ganirelix & full dose Pregnyl) + prometrium (AM & PM) 10/15 --> 1 mature follicle --> BFN 
    TI - Last attempt at injectables (Gonal-F, Ganirelix & 1/2 dose Pregnyl) + crinone (AM only) + vaginal estrace (AM & PM) 11/15 --> 3-4 mature follicles --> BFP!! 11/27/15 @ 13dpo (shockingly, actually waited until then to test)

    Beta #1 @ 16dpo (11/30/15) = 1,075
    Beta #2 @ 19dpo (12/3/15) = 3,150
    One baby: Saw heartbeat @ 5w5d (114 bpm; baby measuring 2.3mm)

    RUN!!  Please don't take fertility meds from an OB.  The HCG is a triggers shot that makes the follicles rupture (ie makes you actually ovulate).  It was smart to not take the trigger, because you could risk multiples with that many follies. 

    Have you had an HSG to check for open tubes?  Has your husband had a SA?

    ETA:  Did your OB have you use the OPKs?  OPKs are not a reliable method to determine O.  Also, ladies OPKs are even less reliable with ladies with PCOS. 

    Also, I have never turned an OPK.  I must have a short surge and drink too much water.
