Femara without IUI- Success?

Hello:
 I am currently seeing a RE Dr who feels that I am not ovulating regularly. He gave me all of the options to move forward including IUI and IVF.I felt pressured to move on to IUI so soon so I said that I wanted to wait.  This past month, I chose to do only Femara and No IUI. Unfortunately I did not get pregnant.  I had my follow visit and sonogram (no cysts- I previous had a couple).  Today, he went back into the discussion on IUI. I still am not sure that I want to do it just yet. I feel like he is just in a rush to get to the expensive IVF treatments.  Has anyone had any success with just using Femara?? I am torn on what to do next.

  • Is he doing a post-coital exam midcycle to see how many sperm are still alive and healthy in a sample from your cervix? If the results are great, an IUI shouldn't be as important. However, if they are not so great, then an IUI is the way to go to improve your chances the most. I believe an IUI increase chances regardless. My 2 monitored cycles, the post-coital test came back with great results, so the doctor said he saw no reason to recommend an IUI. I'm starting injectables next month and am going to ask my doctor about IUI despite great post-coital results as I'm only trying injectables once before moving on to IVF.

    Ultimately, you should go at the pace you feel comfortable. That being said, there are other factors such as DH's SA results and post-coital results that would really make a difference in deciding whether timed intercourse is a feasible course of action.

    Unexplained with (controlled) hypothyroidism and suspected ovulatory dysfunction (but, I do ovulate on my own)
    Clomid 50 mg 3/15 (unmonitored) - BFN
    Clomid 50 mg + metformin 4/15 (unmonitored) - BFN 
    First RE appt. 5/15; Natural cycle 5/15 monitored with 2 mature follicles and Pregnyl Trigger (full dose) + prometrium - BFN
    6/15 HSG - clear tubes & normal uterus; great PCT test results
    TI - 100 mg Clomid + prometrium (AM & PM) + vaginal estrace (AM & PM) 6-7/15 (monitored) --> no additional response and thinned lining - BFN
    TI - Injectables (follistim + Gonal-F, Ganirelix, & 1/2 dose Pregnyl) + prometrium (AM & PM) 9/15 --> 3-7 mature follicles (3 definites and 4+ that could have matured due to trigger) @ O -->BFN + 5 large cysts
    BC for 2 weeks due to cysts
    TI - Injectables (Gonal-F, Ganirelix & full dose Pregnyl) + prometrium (AM & PM) 10/15 --> 1 mature follicle --> BFN 
    TI - Last attempt at injectables (Gonal-F, Ganirelix & 1/2 dose Pregnyl) + crinone (AM only) + vaginal estrace (AM & PM) 11/15 --> 3-4 mature follicles --> BFP!! 11/27/15 @ 13dpo (shockingly, actually waited until then to test)

    Beta #1 @ 16dpo (11/30/15) = 1,075
    Beta #2 @ 19dpo (12/3/15) = 3,150
    One baby: Saw heartbeat @ 5w5d (114 bpm; baby measuring 2.3mm)

  • @Star0418 just curious, is there any particular reason you are not feeling comfortable with doing the IUI?  Besides "I feel like he is just in a rush to get to the expensive IVF treatments."

    TTC- since we got married (off and on)
    TTC with focus: Since July 2015
    Currently had 2 rounds of failed IUIs, failed TI monitored cycle
    December 2015: Starting 1st IVF cycle
    January 2016: Retrieval
    March 2016: Pending Transfer (I have 3 PGS tested embryos waiting for a womb)
    April 2016: BFP, the old fashioned way.

  • No. I really can't put my finger on why. I just feel a little pressured into make a decision.
  • @riveridgional he has not done postcoital testing. It does make sense. My husband's SA tests were all fine. So perhaps it the IUI is not necessary. I will inquire about it when I call the nurse tomorrow.
  • I'm in a similar boat - TTC for 14 months.  I started Femara this cycle and I'm currently on cycle day 6 (days 4-8, 2.5 mg).  I started Metformin (2000 mg) 4 months ago and that has helped me to finally attain a semi-regular cycle.  Fingers crossed for you!  I'm doing 3 months of Metformin and Femara before going to IUI.  Hopefully, we can both share some success stories soon.
  • @bpfeiff1984 thanks!! I am glad that someone understands. I would prefer to try the meds alone before I move to IUI. IUI is covered by my insurance. Part of me just feels that they are in a rush to get to IVF due to the costs.

    Like others say, just do what feels right. A couple of natural, medicated cycles can't hurt and would help you assuage your feelings of rushing perhaps.
