Daycare information

Hi all! I just have a quick question. What kind of information does daycares need of your child. So far, it's immunization records and a physical. I want to be prepared..so is there anything else I should have on hand? Thanks in advance :)

    Ours asked for immunization, a physical (seen by a doctor within 7 days of starting) and then had a huge amount of paperwork with lots of questions about what he could and couldn't do motor skill and words wise. What we were wanting him to get out of it etc. He goes to a Montessori program through so each Daycare is bound to be different.

    It will probably vary daycare to daycare. Ours asked for emergency contact info, who is/isn't allowed to pick up, and our employer's contact info. Mine also requested immunization records (I provide her with updated ones as they come at doctor's appts) so she can keep it in her file.
