Things you may not know about active management of the delivery of the placenta

35 weeks here and just fould out its standard practice at my hospital and many places around to give a needle of pitocin as the baby's shoulders come out. Most women aren't even told and because they are busy pushing or have an epi and can't feel it.

I just switch doctors and told him I am seeking a 100% med free birth no IVs or pain meds. Unless needed and I or hubby consent to it...He informed me that while he is on board I will be getting the needle to prevent any postpartum hemorrhage and that most women aren't told about it. I shutter at the thought that most women are not told they will be having this done and that if I hadn't said anything I wouldn't have been told.

Don't get me wrong I am not against it at all. Just that women are not being told. I understand the reasoning behind the precautionary of administering it but my position is let's wait and see if it's needed.

  • Wow! You should really have a choice in this matter barring complications. You might want to call the head of OB at your hospital to discuss further.

  • I always do active management but I'm always asked about it. My labours are fast and I've had my kids at home with a midwife so I'm definitely comfortable in using it to lessen the risk of PP hemorrhage. I don't think it should be done without consent however.
  • My practice does them after the placenta is delivered to reduce the risk of PP hemorrage. You can opt out. I asked. I also asked if they informed the patient before it is administered and was assured that they do.
  • That's absolutely crazy! Most hospitals have very strict policies about getting consent for everything unless it's a true emergency situation. I can't believe that not getting consent is a standard procedure.

About two years ago I had to go to the ER and ended up having an emergency appendectomy. When I looked over my final bill I noticed that they had run a full STD screening while I was in the ER and charged me for it. Somewhere around $300 after insurance. I called and complained because I never consented or was even told they would be running that test, and you can bet I didn't pay a damn dime for it!
  • I have never heard of this. Interesting.
  • That's crazy that they don't get consent! I've never had the shot afterwards and have refused again this time (unless needed).

  • I knew about that happening before I went in to have my first baby, but no one asked me about it, they just did it. Luckily I didn't mind and was just so happy to have everything out. The birth was med free so I could have been shitty about it if I was that way inclined.
  • I had pit after I had twins and the OB discussed it before delivery. He made it sound like he mainly did it for multiples after delivery because my uterus was so large it needed some help to get it contracted down. I never got the impression he did it as a matter of routine. I'll have to ask now to make sure!
