DOR/ POOR RESPONDING - Letrozole and FSH Protocol (micro dose protocol?)

Has anyone done a micro dose protocol? This will be my 8th and probably final attempt at this. We are attempting this protocol due to my poor responding. I will take the Letrozole from Day 3-7 and then begin 100 Iu's of Follistim on Day 7. I have always been on high dosage of meds from day 1, so this will be a new attempt for me. I sure hope that this can work!

IVF #2 w ICSI-6/21/11 ER, 4 Eggs, 2 Fertilized, 6/24/11 3 day ET 2 embryos- Beta 7/5/11- BFN- No frosties.

IUI #1- started 50ius of follistim 1/25, IUI 2/3/12 - BFN
AMH-0.73- DOR 2/2012

IUI #2- 3/17/12 started 200ius of Follistim , 3/24/12 added ganirelix, 225ius Follistim and 75ius of repronex, IUI- 3/30/12, Beta April 13, 2012-BFN

AMH tested again 1/2013- 1.4!!!!
IUI #3- 1/14/13 started 200ius of Follistim, 1/20/13 added menopur and ganirelix, IUI 1/25/13. Positive HPT 2/6/13- Beta #1- 193- BFP!!! 2/8/13-Beta#2-426.6
2/26 ultrasound #1- TWINS!
Delivered a healthy beautiful baby boy on 10/17/13 10ls. 2 ounces

TTC #2 
AMH .75 VERY POOR RESPONDER
IUI #4 cancelled due to only one egg responding 
IUI#5 cancelled on CD 10,  (3) follicules were still very small even on 300 IU's of Follistim. Also found fluid in my uterus from C-section scar
BFP- 8/9/15 
Beta #1 8/10/15 13DPO-   9.2 (fingers crossed this sticks)
Beta #2 8/12/15 15DPO   39- progesterone 18 
Beta #3 8/14/15    111- progesterone 17.5

  • For IVF, right? I have a very similar protocol for my first IUI cycles starting any day now (5mg of Letrozole CD 3-7, 75IU of Follistim/Puregon starting CD6, for 5-7 days).

  • I will actually be doing IUI this time. I have done it all, but got pregnant with IUI and have spent so much money and not respondingenough for IVF. SO IUI it is!!

  • What are all these medications? I'm in my first cycle and just had to take clomid and ovidrel. What are the others for?
  • @meganfauer :Best of luck to you! Hope this is the magic combo for you!

    @Drove2u : Letrozole is an alternative for Clomid. I can't take clomid because of a history of liver issues (most likely caused by endometriosis as I don't drink much at all) and have a thin lining. So Letrozole (also called Femara) is a better option for me. Follistim is an injectible (also to stimulate the growth of follies, as the name implies!), but it's called Puregon in Canada.

  • No, you still take Ovidrel after. Ovidrel forces ovulation, whereas Follistim encourages the production of more follies (more so than Clomid and Letrozole; it's often used in greater dosage for IVF)
  • My protocol was a bit different but I did two cycles of "mini IVF" due to DOR. One yielded four eggs (which was a lot), three fertilized, put two back in, BFN. Second cycle yielded one egg, BFN. I did a third cycle but did full blown IVF. Still yielded only one egg, BFN.

    I think my egg quality is poor. People do have success with mini. Best of luck to you!
  • I also have DOR and my first IVF cycle was a micro dose. We used Lupron, Menopur and Follistim. I responded well to it and we retrieved 14 eggs, 5 were mature and 3 fertilized. We transferred 2 and received a BFN. Our 3rd embryo did not make it to freeze.
