Gestational HTN & Induction at 39 weeks?

Hi, 
I'm a first time mom and 37.4 weeks now. I have been diagnosed with Gest. HTN at 35 weeks. My pressures have been runnning 130's/80's 140's/90's which is high for me, and when I lay on my left side they go down to 115/60's. I feel absolutely fine. My only thing is my feet are a slight bit swollen. My Doc said no more work for me (ICU RN) since the 35th week so i've been just laying low. I see the doc 2x/week with NST each time (those have been fine). I also do a bio-ultrasound and labs for Pre-E 1x/week and those too have been fine. My baby was measuring about 7.5 lbs at 35 weeks so he said he would like to induce me at 39 weeks if my BP stays as it has been and all my appts and labs have been ok as they have been. 
My question is if I am being induced at 39 weeks and my BP is already that high, I wonder what if any kind of medications they will give me so it doesn't go up any more......i'm anticipating it will b/c i'm going to be nervous and all.

  • I'm not sure if you are referring to medication they will give you during labor or before. I can tell you my experience though, I was diagnosed with GHTN at 34 weeks bp consistently 140/90's then started rising to 169/110's. My doctor put me on labetelol, and wants to induce at 37 weeks. I am currently 36'4. My pressures have been controlled on labetalol and strict bedrest for the past 3 weeks. I have also been doing NST and weekly appointments. Although they want to induce at 37, unless my NST or bp starts being abnormal again we will try to wait for 38 & 4. I don't think your pressures are considered high enough at this point to do medication. But I bet if it starts to rise to around 160/110 they will put you on beta blockers such as labetalol OR if you are past 37 weeks just deliver.

  • I am 27 weeks now and last week they diagnosed me with ghtn and started me on labetolol 100mg bid. My blood pressure were running 140's over 80-90 which is much higher for me and o have been having headaches with seeing spots so they are monitoring me closer but have said if it gets to 160 or higher they will put me in the hospital. So scary being it is way to soon. Hope you ladies are doing well otherwise!
  • Hello, lurker here.  Definitely not in my 3rd trimester, but was interested in your post.  I am an emergency department RN and generally when we have a gestational HTN come in in labor we give IV hydralazine and then IV labetalol (depending on severity of course and the doc, etc.).  Guidelines likely change by hospital/health authority.  Often gestational HTN moms are put on oral labetalol as well.  Hope that helps!  But if you are having any concerns I wouldn't hesitate to contact your doctor to get a full plan from him/her just to put your mind at ease.
  • I'm in the exact same situation, also 37.4 weeks! I'm going basically every 3 days now to the doc. With NST&ultrasound to check fluid each time. I was diagnosed at 22wks with GHTN and thank goodness it hasn't turned into pre-e. Just got finished doing lab work and everything look good so hoping it stays that way. I haven't been on any meds at all and all doctor has told me is they will induce at 39 wks unless my BP gets to the 160s/110s. So we shall see... Just a waiting game as of now. I really hope my cervix is somewhat favorable by the time they decide to induce though to make the process a little easier.
  • I had normal blood pressure until my appointment yesterday at 39+2. There was no protein in my urine, nst and ultrasound were normal, and I feel fine other than slight swelling in my fingers and feet. They took blood to check my platelets but I don't have results yet.

    It sounded like they would have induced me yesterday if I was dilated...I'm still fingertip at best so there would be a higher risk of it ending in c section. I'm going back tomorrow morning...if it's back to normal and platelets were good, I'll go on my merry way...if not they will induce tomorrow most likely. I'm due Monday anyway so waiting it out and medicating me for it is probably unnecessary at this point.

    I had HTN at 40 weeks & they let me go to natural delivery at 41w2d. I just checked my pressures & did urine captures.

  • Thanks everyone. I they are going to wait one more week to induce me (39 weeks). My BP has still been steady 130's/80's 140's/90's. I'm not on any meds. I'm just worried that when I DO go into labor i'm sure it'll go up....so yea, they will probably give me something IV to bring it down. I'm really hoping my cervix is getting ready for this baby to come...I'm was still only 1 cm and 30% on Monday. Today I did a lot of walking so I hope that helps out.   
  • I had it but the ob on call said that she wasn't worried during labor unless I had 3 high readings in a row in the 150s and 160s. Your contractions will raise your bp.
    I think if it was consistently high during labor they would have done a c section.
