Spitting up then pooping

My DD started about a week ago with grunting then pooping and spitting up a lot. Took her to the doctor they said didn't seem like it was nothing. She was ok for two days. Then today she started again eating, grunting, then. Poop and spitting up. It doesn't affect her but it comes out of her mouth and nose. It's more of a laundry issue. Does anyone else have this issue?? And any suggestions??

    My son will do this when he poops while eating.  Normal, just annoying...and gross when it comes out the nose.  If I notice he is trying to poop I stop feeding him and burp him.  If I get a burp before he poops he usually doesn't spit up.
