HCG and vanishing twin? IVF

hi all, just wondering if anyone has ever had this....my husband and I recently completed our first IVF treatment (no known fertility issue on my end just something with my husband). 4 weeks pregnant hCG was 141 (nurse said anything above 50 is healthy). One week later (5 wks) hCG was 1300. Went in 4 days later and my number only doubled once in 4 days (2300)!! Doctor said it could be vanishing twin (since originally they implanted 2 embryos). Anyone ever have this experience with IVF? Had a ultrasound 5w5d and sack was there so no ectopic! Thanks!!

Re: HCG and vanishing twin? IVF

  • Vanishing twin totally happens, but also it's not uncommon for doubling rates to slow down once you hit the thousands. 141 seems pretty normal for a singleton. Since they transferred two originally and now there's just one, there's definitely a chance that there was a vanishing twin.

  • Hi,

I just had my 8week 6 day ultrasound this morning and we have a vanishing twin.  (We are preggo via IVF)

Our 1st u/s was at 6weeks 5 days and showed 2 heartbeats - the first at measured at 116bpm and the second at 114bpm they both had good measurements.

This morning we went for our 2nd u/s and baby A had a 160 BPM heart rate but we were unable to locate the heartbeat on baby B.

The good news is Baby A is measuring perfectly and heart-rate is on track!  So kind of a strange happy but sad day all at once.  Not sure how I feel about it.  Kind of in a fog.

I'm so sorry. I lost twins at 23 weeks. They are truly a special thing. Hugs.
  • On Monday I was released from my RE to my OB & MFM. We had a loss on 3/2 after IVF prior to our first u/s. We conceived naturally this time (surprise! We were trying not thinking it would ever happen,) while waiting for a "normal period" to start cycling again. Monday's u/s at the RE (our 2nd u/s) showed a nice heartbeat at 159.5... I was so relieved but was questioning how we could only be measuring 5w5days at our first u/s with our Betas where they had been at.

So today I had my first visit with my OB. She was doing a belly u/s and was uncertain about what she was seeing. She thought she saw a second sack but could not see anything in it. She said she wanted the MFM to check. Anyhow, she gave me the picture and sent me on downstairs to my MFM (same building.)

Anyhow, the transvaginal ulstasound confirmed that we have/had a vanishing twin. The baby looks good though and had a heartbeat of 150. We now have an estimated due date of 12/18.

I can't help but feel a little sad. I'm trying to focus on the positive and am still thrilled about being pregnant. How can you feel loss over something you didn't even know was there? Anyhow... I'm hoping the little sadness will pass.

It will, I'm sure it will just take time. So sorry for your loss. My coworker went through the same thing with her first set of twins. She was pregnant with two and lost one very early on and she went through the same emotions. Her little girl is now 2 and the light of her life! Obviously she will always remember that there were originally two and mourned the loss, but the joy she gets from her daughter fills her heart too much to dwell on the sadness!

Oddly enough, she got pregnant again last year with twins and now also has an adorable set of 2 month of twin baby girls!
  • @kyleneum13 thank you. I'm sure that you are right and that is a beautiful story about your co-worker. Thank you for your support!
  • @fashiongirlsteph i'm sorry for your loss! I was wondering if the vanishing twin reabsorbed into your system? And if so how long did it take?
  • edited May 2015
@BostonBaby1 I was told by our nurse that it would absorb, that was on April 28th, we were 8w6d.
I don't have another u/s until the 15th of May.  Will let you know when I find out.

  • @fashiongirlsteph Thank you & fx everything goes beautifully at the next u/s! I was curious, how was your emotional state after the news? I'm confused a little about how I feel. Happy that we still have a good pregnancy, a little sad that there is another angel baby, but most of all scared. I'm scared something else will go wrong.

Trying to stay positive though. I think it was just a little bit of a shock for me. Are you a FTM or do you have any LOs?
  • @bostonbaby1

It was a very strange range of emotions especially since we have been going through fertility treatments for the past year and a half and this is the furthest we have ever gotten!   We had a mc last year at 6w.  My nurse assured me that this is very common and it is sometimes your bodies way of telling you that it is better off just carrying 1 baby to term.  I had my 3rd u/s this morning and baby was moving all around and measuring perfectly!  You could still see the second sac on the u/s which was sad, however, we are feeling lucky to have made it this far.  I'm starting to let myself finally believe that this may actually happen.  I've been very protective of my heart and like you, sometime fearing the worst.

I am a FTM at 41 years young!  Good luck to you and your little one, keep me updated!  We have our NT scan next week.  I am 11w2d today.

  • I'm 11 weeks 3 days. I had an ultrasound at 6 weeks 5 days that showed 2 sacs but one quit growing 5 days previous. My hcg levels tripled in 3 days and had vanishing twin. I went last week for ultrasound at 10 weeks and the 2nd sac was gone already.

Thank you so much for the update sweetheart! I am so incredibly glad that everything is going well for you!!! How exciting to become an FTM! We are also AMA… Unfortunately, it will be a little bit longer until we are able to have a LO because we lost the other one on Friday. I guess the good news is is that I don't have to worry about the other sack being absorbed. We will keep trying though, I'm gonna have my D&C scheduled on Monday. And then it is back to the RE for us. I will still be lurking on the board for a while so please do keep me updated on what happens at your next scan?! So extraordinarily thrilled for you!

That must be a relief! Thank you so much for the information. Sending you prayers and lots of good luck for your LO

I am tearing up reading as I read this now.  We have also suffered a few losses during this journey and I am so sorry you are going through this.  Sending prayers to you and the rest of your family.
