High-Risk Pregnancy

Cervical Checks

Quick background:  At 13 weeks, I was diagnosed with a short cervix measuring 1.8cm.  I had a cerclage placed immediately the next day.  At 18 weeks, my blood tests came back with an elevated AFP reading.  I had a sonogram at 18 weeks and my cervix was measuring 2.2cm.  I had a MFM appointment at 21 weeks with another sonogram, and again my cervix was measuring 2.5cm.  At that point, my MFM determined that they could not see anything wrong with the baby, but that I was at an increased risk for pre-eclampsia and IUGR due to the elevated numbers.  I also started progesterone suppositories at that point.  I had a growth scan at 26 weeks and my cervix had remained at 2.5cm.  I saw my OB the next day and it was suggested that I come in at 28 weeks (today) for another cervix check, since my next sonogram won't be until 34 weeks.  I went in today and they said my sonogram should have been canceled, that after review, the 34 week appointment would be fine.  I am kind of concerned of going 8 weeks between cervix checks, as this is the most critical time for preterm labor.  I know that the progesterone should help keep things where they are, along with the cerclage, but does this seem right?  The nurse said if I wanted, they could do the sonogram.  If it's not needed, then I'm ok with that (and it saves me a copay), but I can't help but worry that something could happen in the mean time that could have been prevented.  Is it standard not to have cervical checks between this time with an incompetent cervix?

Re: Cervical Checks

  • Definitely ask for a sonogram! Your cervix was quite short and I am surprised you don't have regular check-ups.
For me, the shortest it was was 2.3 cm. Then after the cerclage it doesn't go lower than 3.2 cm, and they still check it every 2 weeks, and will keep doing it until 37 weeks.
Please do ask for your sonogram!

  • She did do a manual check and said I wasn't dilated and my cervix was still firm. My next appointment in 2 weeks is with the actual OB, so I'm going to double check with her (unless they're just going to do manual checks in between).

  • Manual check is better than nothing, for sure. But it won't tell you how long your cervix is.... Well, hopefully it's behaving! And it's a great thing you are already close to 29 weeks, that's so reassuring. Good luck and keep us posted!
