Opinions please!!

37w+ 3d- second child. First time with gestational diabetes. My first weighed 7lbs 9ozs. During labor he got stuck(shoulder dystocia.) after 2 hours an different ways of pushing he finally came out.. Now with baby 2 they said he already weighs that much if not bigger... At first she was wanting to do a CS. But last appt she said I can try to do natural.. I'm scared of him getting stuck like my 1st. Anyone been thru something like this??

  • My last baby was born at 37 w+ 5 days and he was 9lb 7oz i found out I basically was pre GD..I am now 38 w+ 2 days with my 4th child and have GD. My OB didn't even want to discuss a CS but I am terrrified this baby is going to be bigger.my last one was so bruised and swollen and banged up he had some issues coming out but the dr was able to get him.... If this one is bigger I can only imagine how he is going to come out. I'm in the same boat as you except we didn't talk of a CS.. I wouldn't mind having him natural because a csection kinda terrifies me but I'm also scared he is going to be hurt trying to come out the naturally way! If I haven't had him by Monday they are going to induce me.... Best of luck to you!'
  • My first (DD) was born at 40 weeks, 7lb 5 oz. My 2nd (DS1) was born at 38 weeks 8lbs 8oz with shoulder dystocia, though he came out quickly after the RN basically put all her weight on my belly. I am 35 weeks with DS2 and I will have an ultrasound at 36.6weeks to attempt to determine weight. Doc says I can try and push as long as he's not like 11lbs- who wants to push out an 11lb baby anyway?! I had one doc say that big doesn't necessarily mean dystocia and another that said if you have one it's possible it could happen again but things are all stretched out down there and hips are wider. I'm scared he will get stuck also, but I would rather try than have a cs with my third. Good luck!
  • goodness...with a history of actual shoulder dystocia - I'd probably opt for a c/s.

    How do you know this baby is bigger?  Have you had a level 2 ultrasound at an MFM/peri office?


    I vaginally delivered an 11 pounder in the OR as a precaution b/c we knew he was that big - but I had a "proven pelvis" in delivering a 9 pounder with no issues.

    Our "rules" for attempting the 11 pound vaginal was that I would only be allowed to push in the OR, I had to have an epi in place in case of emergency c/s needs, and there would be no forceps or vacuum - if baby didn't drop on his own we would switch to c/s.

    I also labored down for an hour after reaching 10cm, so the team could see if he would fully drop into the birth canal without being forced to.

  • @CivilMommy - you are brave! I am not sure what I will decide to do if he is 11lbs. I really, really don't want a CS- maybe I will think along the lines you had with delivering in the OR. Just wonder what they would do if his shoulders got stuck. Kinda late for a CS at that point no?
  • If it was me and I had a history of shoulder dystocia- I would go for he C- section.

    well if you had shoulder dystocia with an 8 pounder - trying an 11 pounder would be reckless.  The only reason they let me try was because my 9 pounder was "easy" and the 11 pounder's head measurement on u/s (with an MFM clinic) was not much smaller than the belly (head was 15.5" around, belly was 16 or so).  They figured that if I could get the head out, the rest would follow - and it did.  (The belly measurement is the only way they can gauge shoulder width on u/s - so they compare head to belly...if the belly is a lot bigger than the head, then the likelihood of shoulder dystocia is much greater.)

    If the head comes out and shoulders get stuck...I think the options include pushing the head back in to do a c/s - OR other maneuvers that may result in breaking the collar bone to get baby out vaginally.

  • My brother was 10 lbs 1oz. They had to break his collar bone to get him out and now has nerve damage in his right arm. He's 26 married and happy but has very limited use in his arm and its noticeably smaller.
    I do know a few that have had 11 pounders just fine. I guess its just a chance you take.. Myself personally, would choose to be sliced open and battle scar tissue than have my babies collar bone be potentially be broken. Its your desicion in the end tho, not us strangers. But u asked opinion!
