To IUI or IVF?? Opinions Please

Tomorrow we have a follow up with RE to discuss which treatment to try once I have my fibroids removed. What I am worried he will do is say "You could go either way" because that's what he said about removing my fibroids. So here is all our info-DH has 0 sperm besides 12 vials of frozen stuff from 1996, I have a blocked tube and we will be paying completely OOP for whatever treatment we seek.

So if this was you and your RE said "you could go either way" which would you do? Also, is there anything in particular I should ask RE tomorrow when we meet? I'm sorry I'm just new at this and nervous! Thanks for your help!

  • Honestly, with limited sperm I would jump to IVF.  IUI has such a low success rate, I wouldn't want to waste it.  :)  Good luck!

  • I agree, with the limited sperm plus a blocked tube I would probably go straight to IVF.

  • Agreed with everyone else. Since you only have a finite number of sperm and a blocked tube I would go for IVF as well. Good luck!

  • honestly? i think you should try at least 1 IUI.

    Did DH have sperm count issues when these swimmers were frozen? how many millions of sperm are in each tube? IUI requires only min of 10mill and if his swimmers were stellar your sperm should be great.

    you have a blocked tube. okay... but there are tons of studies out there that illustrate how in more than 1/3 of fertilizations (in the studies) the tube OPPOSITE the dom follie picks up the egg. maybe you can do IUI+meds to increase your chances of an egg on both sides thereby increasing chances of success...

    also, IVF is a bitch. it's crazy expensive. it's crazy hard on your body, mind, and spirit. and seeing as you have no egg issues (none listed in your siggy) and im assuming DHs frosties are fine i think it's premature to jump right to IVF.

    but that's just me. having gone through a bunch of shit myself.

  • This is a tough question for sure.  At first, I wanted to say jump to IVF but I guess I would ask more questions.  How much sperm is necessary for IUI? How much for IVF? (I.e. do you have twelve shots at this or like 3 total?)  If you had more than like 4 attempts I might go IUI too.  It is easier on you and if it works would be great!  If you only have 2 or three, I would say IVF.  Although I hope no matter which it works right away!

  • I would probably go straight to IVF with ICSI.  It is tough on your body and it is more expensive and it still is not guaranteed to work.  However, you have a lot of cards stacked against you with only frozen sperm and one tube.  IVF would completely bypass the tube issue.  ICSI would help if the sperm wasn't as active when it thaws.  Additionally, you will use a lot less sperm doing IVF with ICSI than you will doing IUI.  I would also ask if they are able/recommend refreezing a vial if it has to be thawed and only part of it is used.

    Me= 37 and DH = 41 

    Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)

    IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN

    IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.

    IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132.  Lil is here!

  • RE wants us to do a round or two of IUI after my surgery. His thoughts are that my tube is blocked because of fibroids so hopefully that will get cleared up after surgery but even if it doesn't he thinks we have a good chance with IUI. DH's frozen vials look REALLY good (of course because they are from when he was 20 yrs old!!) and per vial there are 15 million in 6 and 8 million in the other 6. My ovarian reserve is very good as well which is why he thinks our chances of success will be good with IUI, but of course he said if we want to go straight to IVF instead he will do that. I told him my biggest worry is running out of sperm using it on IUI and he promised me he wouldn't let that happen.

    So once again I'm torn and a bit overwhelmed. DH agrees with RE but hates the fact that we are probably going to have to wait another 6 months to even do a cycle. He just said to me "My sperm is the real problem with us getting pregnant not your fibroids so I saw we just get the frozen ones out here and try IUI instead of doing your surgery" He's making my head spin!!!

    Ok if he really said this, your Dr is crazy. First of all, for him to imply that your tubes being blocked "may" resolve after surgery, "but even if it doesn't" he thinks you still have a good chance with IUIs is downright INSANE. How could you get pregnant with IUI if #1 the sperm can't pass through the tubes, and #2 EVEN IF IT COULD PASS THROUGH THE TUBES, a fertilized egg can't pass through blocked tubes?!? Did he get his degree from inside a cracker jack box, because all you need is common sense to figure this one out?! 


    RE: Sperm counts in your frozen viles.  Those numbers are NOT great. No where near it. Now if someone was just dealing with low count and no other issues I would definitely say give IUI several tries, however that is NOT the case with you. This is your ONLY SPERM. The fact that this Dr is encouraging you to potentially waste any of it on a IUI is MFing NUTS. 


    I'm not sure if your Dr is just trying to be optimistic but he is definitely not being honest with you, in fact I would go so far as to say he is being overtly negligent, considering you have such a limited supply of sperm to work with. Great counts are up in the 100s of millions, most clinics like to have 30mil + POST WASH for IUIs. 

    Can you get a second opinion??

  • I'm going to second @potatocastillo‌ here, and mostly because those aren't great numbers for sperm for IUI. Fwiw I did seven IUIs (2 resulted in mmc) and H always had at least 35 million post wash, sometimes 100 million, and all other numbers in normal range. IUI is a gamble, it increases your chances some, but really not that much, and with the limited sperm, which still needs to go through thawing, I'd jumped to IVF with ICSI and not look back. Yeah, like @Jodee37‌ mentioned, it can be stressful, but so can IUIs imo. I'd take the stress with the highest chances!

     because if both were blocked i can see how everyone says jump straight to IVF. but if one is open and she has good ovarian reserve and sperm counts are good why on earth would you go straight to IVF with ICSI?

  • Because you don't want to kick yourself later. Too much of a risk. And sperm counts aren't great. And correct me if I'm wrong, but although the unblocked tube CAN pick up an egg from the opposite ovary, it does lower success rates some, right?

    I'm also keeping in mind that @NB817‌ might want more than one more child. So more reason to not risk wasting sperm.

  • I have different issues, but my DH's count was 85-89 million post wash for all 3 of our IUIs. My doc said even under "perfect" conditions the success rate of IUI is only 10%. If I was in your shoes I think I would try one IUI and then move to ivf. Just so I could say I tried a less invasive procedure but not waste too much time. Then again since you are OOP you're spending extra money instead of just investing in ivf.

    Repeat c-scar surgery June 2015
    2nd and last IVF cycle August 2015, stimmed for 12 days, 2 egg retrieved, both mature and both fertilized.  Transferred both 8-cell embryos on Day 3, beta 9/5/15 = BFFN

  • You only have that finite number of vials and you might want more than one child.

    IVF/ICSI, and if you have good ovarian reserve then maybe you will end up with the some frosties too.

  • Additionally, the RE at the "big clinic" I just met says that frozen sperm has a 10% less chance of working in any procedure since the sperm's integrity can be compromised in the freezing than thawing process it can cause DNA fragmentation.  Just something to consider.

    I have to say that with all the information you've given, I too would say go straight to IVF with ICSI.  Limited amount and total number of sperm and blocked tube, I wouldn't spend the time, money, and emotions on IUI with only a 10-12% pregnancy rate.

    ETA: And with good ovarian reserve, the chance of having frosties for later is good whereas you can't get that with IUI.  Later down the road, a FET is only slightly more expensive than medicated IUI and you will have the benefit of your embryos being "frozen in time" at whatever age you are now.

  • Wow. Okay. Thank you for the responses and I am once again overwhelmed.

    To clarify RE said that if we went to do IUI and my left tube was still blocked and the egg was on the blocked side we would skip that cycle. The reason he is thinking/hoping the tube will be unblocked after surgery is because he thinks its blocked due to the fibroids that I will be having removed but of course we won't know that until after the surgery happens. I don't think he is a cracker jack or anything like that and I trust him. He did say that he expects the thawing process will make us lose half of each vial.

    Those 12 vials consist of 2 sperm samples DH gave when he was 20 years old. So it was 90 million and 48 million of post washed sperm they froze. DH had testicular cancer when he gave these samples so they are what they are and I can't change that.

    We do want only one more child...if we were to end up with twins or more because of IVF we would feel blessed, but the goal is one more.

    We were given the cost breakdowns for IUI and IVF and it wasn't as horrible as I thought...IUI is $600 with monitoring and IVF $10k not including meds which I realize can be the really expensive part, but again it is what it is.

    Thank you again for the responses and yes I've got a lot to think about in the next few months.
