Plugged duct- when to call the Dr?

I've gotten a few clogged ducts before from pumping but as soon as I nurse they clear out. I've had one now for almost 2 days. I've massaged, nursed, pumped, put on hot compress... still got it. At what point do I call the Dr? When does it turn to mastitis?

Hold a heating pad to your breast for a half hour prior to pumping. It helps. I finally got my plug to clear after nearly 3 days. So painful. It felt so good when it emptied.
