Intro - right place?

Hello everyone, I'm not 100% sure this is the right place for me, but I will introduce myself and if I need to go somewhere else, I will :)

I have 2 children. My daughter will be 4 next week, and my son turned 2 in early December. I had high risk pregnancies, preterm labor, and all kinds of issues. I had 3 losses before my daughter, 1 loss last year, and 2 losses this fall when we started actively TTC again. I am in remission from thyroid cancer as of August, which is when my doctors gave us the go ahead to TTC again. I had a CP in September and a miscarriage at 5 weeks in October. My doctor ran an RPL panel after my last loss, which showed that I have APS (antiphospholipid antibody syndrome) which my doctor believes is causing me to prevent proper implantation of embryos. I also have Hashimoto's disease (autoimmune thyroid disease), Reynaud's disease (another autoimmune causing circulation problems in hands and feet), and chronic Lyme disease. I am a student nurse. I'm currently on 40mg Lovenox, 81mg Aspirin, 175mcg Synthroid, and 100mg Zoloft daily. I am on 600mg progesterone daily starting at 3dpo, split into 200mg three times daily. It's compounded progesterone vaginal suppositories, the bullet kind. I did 100mg Clomid cycle days 5-9 and produced 1 follicle (22.5mm) at my scan on CD 15 but I didn't ovulate so I got 10,000 Novel on CD 18 and ovulated on CD 19. I started progesterone 3 days later but spotted from 7dpo to 10dpo and started a new cycle today. I was going to start 2.5mg letrozole twice daily (5mg per day) from CD 5-9 but for reasons listed below, I am holding off.

I just got accepted into a clinical trial in St. Louis for recurrent pregnancy loss. Our consultation is January 8th. They will do an antral follicle count and blood work on me, and a DNA fragmentation semen analysis on DH. Our karyotyping came back normal, my HSG came back normal, and everything except for my anticardiolipin antibodies came back normal, which lead to the APS diagnosis and lovenox & aspirin. We aren't sure what we will be doing in the trial. It could be ovulation induction with oral meds, injectables, monitoring, etc, or IUI, or IVF. We will find out at the consultation.

  • Welcome, and sorry for your losses. I'm curious what you've been told about your Reynaud's affecting IF? I have it, too, but it's never come up with my RE.

    Two DDs 10/08 and 08/10, no primary IF

     3 BFN clomid + TI cycles, 5 BFN clomid/gonal f IUIs, 1 mmc IUI
    2/19/2014 IVF #1 Unexpected low E2 (oversuppressed) -> increased to max doses = 3 or 4 follicles, converting to IUI
    BFFP Saw 1 beautiful heartbeat at 6w6d, follow up u/s at 9w showed mmc. Eff this.
    NTNP 5/2014-9/2014, OPKs and TI 10/2014 - 1/2015. 
    RPL testing all normal, AFC, AMH, and FSH all normal. 
    IVF 1.2 1/22/2014 natural cycle start, AFC 28, 300 gonal f/150menopur. 
    ER 2/3/15 14R 8M 3F w/ICSI Day 5 transfer on 2/8/15 of one "Grade A+" blast and have TWO frosties!

  • He never said anything about it affecting IF :) I just listed it with my AI diseases because once you have one, you're more prone to others. That's part of the reasoning my Endocinologist used when explaining why I have so many and why he will periodically test me for more if I present symptoms. I think the Reynaud's is the least of my worries by far haha!

  • Thanks ladies! I was very shocked to get a call back saying I was in! Hoping the consultation and prelim tests go well. So I know my ticker changed. I'm pretty confused because I thought AF had started. I bled for 2 hours, lightly, but I needed a pad instead of just spotting when I wiped. I cramped all day, and then stopped. I haven't bled since then, but I haven't been taking progesterone since then either. I'm now 13dpo and 14 days past trigger, and I got a BFN this morning, but still no sign of AF, not even spotting. Do I just keep waiting? Do I call my doctor and ask them to do something? I've never used anything besides progesterone to induce a new cycle, so I'm not sure what to do. Obviously, I'm not pregnant. It would have shown up by now. I'm charting BBT and I took OPKs and had a trigger shot. Unless I had a clear temperature shift without ovulating, then I'm sure of when I ovulated. This is incredibly frustrating. I just want AF to show up so I can start my Femara.

  • And just like that, we may not be doing the clinical trial at all :(

    I called the office doing the clinical trial to ask about what is covered and what is not. They said they will cover the NKC test and immunological blood work, but not the CD 3 blood work or the AF count ultrasound for me. It's $150 per ultrasound (and they want to do CD 4, CD 11, and any more as needed) and didn't give me the pricing on the CD 3 blood work. DH's SA and DNA fragmentation test also aren't covered, and the discounted price is $400. Both of those are their discounts for OOP pricing.

    We just don't have that kind of money. I'm a full time student nurse, DH's sister (lives with us) is a part time student but we are paying $700-800 per semester for her because her classes are out of pocket as she doesn't qualify for financial aid. DH is a full time student (covered by tuition assistance for active duty) and works full time but since he is Air Force, he doesn't get overtime or anything. It's a set pay. And that's ok, we just don't earn enough to even set aside remotely that much right now, much less for more than one cycle. It would drain our savings completely, and we can't risk that. I mean, something could happen to the car and need a repair, and we would be out of money.

    So just on a whim, I asked if my OB was willing to do the AF count u/s and any other needed u/s, CD 3 blood work, and the SA & DNA frag test for DH, if they would be willing to take the results from the OB since it's covered if done on base, but not if billed by a fertility specialist. They said yes (wow! yay!) so I called and explained to my OB's nurse what the situation is and asked if Meinhart would be comfortable working alongside with Ahlering for the clinical trial. His nurse said since DH has never had a SA or DNA fragmentation tested before, that would most likely be something the base could do anyway, and that she will talk to Meinhart for me and explain the situation and see if he is comfortable/willing to work with Ahlering. If not, I'm just going to stick with Meinhart. He has been great, it's covered on base, and we can move on to other treatments in the future if needed. So now it's just a waiting game to get a call back and see what my OB says. If he says no, I will just schedule DH's SA and my CD 3 blood work and ultrasound with the base and move on as usual.

    A little frustrating, but oh well.


    Sorry for the book. There's just a lot happening for me and I'm feeling overwhelmed and needing to put it out somewhere.
