Does this sound like thrush?

I have my PP appt tomorrow and will ask but I want to arm myself with info first. Twins are almost 7 weeks old, but C was tongue tied and has only really been nursing since he got it clipped almost four weeks ago so I am looking at it as though I've only been breast-feeding for four weeks rather than seven. At four weeks with my first we were still dealing with sore nipples but it was better at six weeks so I am giving myself two more weeks for the sore nipples to get better.
They are very pink, but not shiny or flakey. They hurt at the initial latch but not throughout the feed unless the latch is poor bc they (or me) are sleepy or lazy. The initial pain is like a burning pain. After BFing they hurt for a while after, maybe 20 min. The same burning pain. I get vasospasms- blanching and pain. But I'm wondering if it's all from trauma or could be thrush or something. I have been rinsing my nipples after feeds and letting them air out, and using coconut oil the past day or so which seems to be helping. Should I ask for APNO if it's not thrush?
Eta; no signs of thrush in LOs.

  • Thanks RN. The only thing that makes me wonder if it is thrush is the pink nipples. I just tried out my new smaller breast pump flanges and it was much more comfortable so I think maybe that will help with the pain and maybe it is all just damage that will resolve.

  • My pump flanges are too big and my nipples are very pink and sore. I am ordering smaller ones today so I hope that's what it is.
  • I'm currently on my second round of thrush in 9 weeks. You don't have to have shiny nipples. Pink with a burning/stinging sensation before and after feeding is indicative of thrush. My first round with it at 1.5w PP I had it all the way back into my ducts and that hurt while feeding. Now I caught it early and it doesn't.

I have a mixture of 1TBS white vinegar in 1 c water that I spray on my nipples and wipe off post feeding. Then I apply a smear of Lotramin which I wipe off with water right before I use that side again. It can take a couple weeks for thrush to resolve so you have to keep at it.
