Your contract or company's HR policies should have information on this. You'll need to approve your time off with your boss/supervisor, and he or she should be able to help you file the appropriate paperwork with the HR department.
You don't need to do this now. Most companies have a policy of 30 days before, so I wouldn't think you need to worry about this until at least 60 days out from your due date.
Yes, I would advise talking to HR ASAP so you can plan financially. I work for a pretty large company, and was surprised to learned that our maternity policy is entirely unpaid. Fingers crossed that yours does offer some sort of a percent of salary, but I wouldn't hold my breath.
