Trouble TTC

First trigger tonight..

So I'm triggering for the first time ever tonight. Ovidrel. I'm a weeee bit nervous. H has never given an injection before.

Anybody have any advice?

Also.. How long after the trigger is ovulation usually?

  • I did my first one last cycle and I was super nervous too, but it was totally fine!! I ended up deciding to do it myself-- at first I thought that it would be better for H to do it, but as it came closer to the time to actually do it I started thinking that I would feel more comfortable if I was in control. He stayed in the room with me and read the step-by-step instructions while I did it. It barely hurt at all, just the tiniest pinch, and was over before I knew it.

    2 HSG tests: Right tube is blocked, possible endo.

    TSH elevated, started Synthroid 25 mg daily.

    October, 2014: Femara 5 mg + TI ---> 3 follies on blocked tube side ---> BFN

    November, 2014: Femara + Ovidrel + IUI#1--2 follies (on the good side), 46 mil. motile sperm=BFN

    Nov-Dec 2014: Femara + Ovidrel + IUI #2 (1 follie, 76 mil. motile sperm) + Endometrin=BFN

    January, 2014: Femara + Ovidrel + IUI #3 (1 follie, 38 mil. motile sperm)=???

    New RE appt. scheduled for 1/14.

  • You'll be surprised by how little you'll feel. Probably the smallest pinch on the way in (way less than a blood draw) and nothing after that. Of course this depends on your pain tolerance, but the needle is so small. Good luck!!!

    They say you'll ovulate anywhere between 24 to 36 hours after trigger. Are you doing TI or IUI?

    July/August 2014 - Testing complete: Testosterone & AMH very high, FSH slightly high, Vitamin D low, tubes and lining all lovely. DH SA: A+

    Cycle 1 (Nov 2014): 2.5 mg Letrozole/Ovidrel/TI = BFN

    Cycle 2 (Dec 2014): 5 mg Letrozole/Ovidrel/TI - BFN

    Cycle 3 (Jan 2015): 5mg Letrozole/Ovidrel/TI - BFN

    WTF consult scheduled for 1/29

    TI .. I was told to BD today tomorrow and Tuesday and then every other day for a few more to be safe.

    Will an opk be positive when before I ovulate after I take the trigger? Or will the Hcg give me false positives.. Since I'm hearing 24-72 hours for ovulation how will I know when I ovulate?

  • I'm not sure about OPKs (I don't use them because...you know...PCOS) but I would say have sex every day if you can since I assume SAs were fine and you don't have to "save up" (sorry, I'm mobile so I can't see siggy). Just to be safe.

    The Ovidrel shot is better than the Nuvarel! I had to give myself the Ovidrel last month and it was easy. I did get a little nervous right as the needle was about to go in, but it doesn't hurt, only pinches...and I didn't have any tenderness afterwards in the injection shot. 

    This month, my RE's office gave me the Nuvarel shot, which is intramuscular, and that injection site has been sore ever since! (I got it Friday). 

    I've been told that the HCG injections typically induce ovulation 36 hours later, as PP have stated. I've never had a positive OPK after the trigger shot, but my cervical position was up and open 36 hrs later, so I'm pretty sure it did its job... GL!!

  • Good luck tonight! You'll do great. When I did my first one last month, I barely felt it go in (I had my friend do it who went through fertility treatments before). I had a bad reaction days after but the actual shot was a piece of cake.

    Some pointers. Ice the area before, make sure you know the directions before (read them over a few times before you do the actual process), and inject into a very "fatty" area. If you don't have a lot of a "fatty" area then make sure you gather a good amount of an area in a squeeze lol.

    Lastly, I found it helpful to buy HPT cheapies and test out the trigger. It put my mind at ease so I knew when it was out of my system. But, that was just me :)

    thank you :) i have a bunch of wondfos i will definitely be testing out trigger!! plus it will give me an excuse to test everyday haha. thanks for the pointers.. luckily i am a nurse so i know how to give a sub q injection.. i am just nervous it will be on me! and now i dont know if i will do it myself or if i will still have H do it haha

    Pp covered a lot. I did my first trigger last cycle and am terrified of needles and blood and it really wasn't bad at all! Just try not to over think it. Good luck!

    Me: 23 Low AMH and damaged ovaries due to chemotherapy.
    No AF or O in 3 years. HSG showed a slight T shaped uterus.

    High Risk OB 9/29- got the ok to get pregnant.
    RE Appt:  10/28/ U/S showed follicles, but also small damaged ovaries.
    B/W results CD0: all normal except low AMH at 1.3
    Cycle 1-November (TI)- Femera 2.5mg, 2mg Estradoil, and Trigger=BFN
    Cycle 2-December (TI)- Femera 2.5 mg ,4mg Estradoil, and Trigger= No O
    Cycle 3-January (TI)- Femera 5 mg, 2mg Estra

  • Glad it went well. I get the same temp results as @BlueFairy5, and I have always been able to pinpoint O even with Clomid and the trigger messing with things. Fingers crossed for you this cycle!

  • Thank you all! I forgot to temp when I woke up this morning but I will definitely temp next cycle! I have a feeling this cycle isn't going to work since I am O'ing so late.

    I did use opks this morning just for fun and it was cool to see positives for the first time ever even tho they are from the trigger. Lol

  • Congratulations!! Good work. :)

    I temped the first time or two on Femara and it worked fine, but I stopped because it felt unnecessary with all the monitoring.

    What I've read (and told by our RE) is that ovulation is 36-39 hours after trigger. That's why IVF egg retrievals occur at 35 hours past trigger - it's pretty accurate (but now I'm thinking they might use a different trigger?). Of course, O is not an instantaneous event - ultrasound will show a slow leaking of fluid as the follicle ruptures and slowly shrinks up, and at some point in there the egg is released. The egg is not chemically penetratable by sperm until about 6 hours after O. So there is a range of timing in there. Highest success with B2B IUI comes from 24 and 36 hours (good to have sperm waiting) and there is slightly lower to similar chances (depending on the study) with single IUI at 36 hours past trigger. Perhaps timing TI similarly is a good idea.

  • Sorry to quasi-hack your post, but I'm about to start my first fully medicated cycle and just wondering, is $80 from a local specialty pharmacy about the best I can probably get on Ovidrel? From what I've seen, it seems to be in the $100-120 range normally. I'd pick it up, so no shipping.
  • I paid 84, and I think that sounded like it was the cheapest around unless insurance covers any of it.

  • I don't have anything to add as I have not done that yet, but I wanted to wish you GL! 
    Me: 25 Dh: 25 Married since July, 2011
    Diagnosed with PCOS 2010
    TTC since December, 2011 (SA is Normal)
    2012-tried natural w/Metformin 1500 mg
    11/12 -Saw an OB, bloodwork revealed everything normal except for highish blood sugar levels
    1/13: Clomid 50 mg - No response
    2/13: Clomid 100 mg O'd BFN
    3/13: Clomid 100 MG O'd BFN
    4/13-6/13: Clomid 150/200 mg O'd BFN 
    Stopped treatment because of money issues and began to try naturally again from June-October 2013
    Benched until November 2014 - Started seeing RE, discovered that lining was very thin
    November 2014: Started Femera 5 mg -No response BFN >:(
    December 2014: Upping Femera, injectables are the next step if I respond 
    Also: Changed RE, first appointment on Friday, so treatment is subject to change this month

  • Thanks! No insurance for the Ovidrel, hopefully they'll cover the rest though since they categorized me as "ovulation disfunction" for insurance purposes. Aaaannd Walgreens just called and it looks like they probably won't cover the Letrozole either but that shouldn't be much. 
  • Good luck!!  It's not bad at all.  I triggered in my thigh last month and it was easy.  Just count to 3 and go for it.  :)
  • I only paid 80$ for my ovidrel I got it from freedom pharmacy they mailed it to me. I got my femara at a local drug store it only cost me 4$ since it's not technically a "fertility" med
