Julia had an awful diaper rash that ended up being a yeast rash so regular diaper cream didn't help. But when we thought it was just a normal rash, the pedi recommended putting on a layer of Butt Paste and then covering that with a layer of petroleum jelly so the diaper cream stays on the skin and doesn't just rub off on the diaper. She also recommended sitting her in a warm bath for 10-15 minutes a day and then letting the area air dry before putting her diaper on. As much diaper-free time is best but sometimes hard to do. I know a lot of moms recommend Triple Paste but I never tried it.
Have you tried the extra strength Desitin? It is much thicker & I noticed a difference when I got it. I bought the extra strength (purple) Desitin & I put that on thick... then a little sprinkle of baby powder. It really helped DS2 whenever he had diarrhea from being on an antibiotic. More frequent baths help a lot too. We usually do every other day baths here.
My pedi recommended Triple Paste when LO had a really bad rash, but it actually made it worse for him. I tried Bag Balm (it's not a diaper rash cream, more like a petroleum) along with no baby wipes for a day and it cleared right up. Now I swear by bag balm for LO's bottom.

That's what we ended up using on Julia's yeast rash. Neither prescription cream worked for her but the lotrimin worked almost immediately,
