I need advice please

Ok so I'm mostly just a lurker that sits and reads all the posts.
My daughter Caroline was born November 16 and we have all sorts of trouble with breast feeding first she wouldn't latch because of flat nipples so we using a sheild now and she latches great because of it.
Then my milk took to long to come in and she went from birth weight of 7lbs 10 oz to 6lbs 7 ozs and her dr and my lactation consultant wanted me to start supplementing formual after each feeding to get her back up to birth weight. Which is fine I just want a happy healthy baby. I asked her doctor about nights if it was ok to let her sleep or if I needed to wake her to eat and he said that she is gaining weight now so the more we can sleep the better. So our current rutine is eat at the breast for a hour which when she is done I still see milk in the sheild some times pooled then formula feed from bottle average 2 oz or so then she sleeps anywhere from 3-5 hours. Is this normal? I try to pump after some feeds as well so I don't have to use formula to supplement. Her dr is happy with her weight gain at her last check in and she has a good amount of dirty diapers. My lactation consultant wants me to wake her every 2-3 hours to feed for a hour then pump for fifteen minutes after I can't do that I have a four year old and still need to eat. So I guess my question is am I doing this right? Should I wake her more often? She gets tired so quickly if I wake her and won't be at the breast long enough. Then just fills up on formula. And the hour on the breast is killing me. I just want to make sure I'm doing this like I should. Thanks for your help in advance because I don't know where else to turn.

  • I'm far from an expert but an hour seems really long to me. I won't typically go longer than 40 mins. Is that one side or split between 2? She might be comfort nursing rather than actually eating that whole time.

  • It's just on one side. Then I pump and still get more out
  • She falls asleep a lot during the feed if I try to put her down she wakes up and starts again and she is swallowing. Then she takes down that bottle in like 5 minutes. And yes they want me to feed so much and pump to get my supply up but I don't think that the issue because if I don't pump after I'm hurting.
  • I do use breastmilk durning the day for supplementing. I wish I could switch my lc but that's who the hospital has that is paid for by insurance. We did try the sns but we can't use it with the sheild. We did do a weighted feed and it said that she only got a half an oz in about a half an hour.
  • Sooo--I used a shield for the first 2 weeks bc she couldn't stay latched on bare breast. It meant very long feedings --like almost an hour.i also supplemented with pumped milk at night so she would stay asleep for almost 3 hours. It takes Ionger on the shield bc your nipple isn't as stimulated and doesn't produce as much milk--which is why they want you to pump--more stimulation to keep up your supply. I went and saw ANOTHeR lactation consultant who is an IBCLC (the best certified an LC can be) who found a lip and tongue tie. We got both revised by a dentist who does water laser. Same day she could latch bare breast. Now our feedings are 30-40 min and she sleeps ALOT more! I would suggest getting to an IBCLC even if you have to pay out of pocket--and one who is trained in finding lip and tongue ties. Good luck! I hated the shield!
