Success after IF

Assisted Hatching

After talking to the nurse at my RE office about this past cycle and being told that our embryos did not look very good this time.
They are attributing the poor embryos to a sperm issue. We are MFI, my husband has a blocked vas deference. We only have his frozen testicular sperm to use, and apparently they are not real "go getters".
I was wondering if anyone has used or is knowledgeable with assisted hatching, does this help with low mobility or immature sperm?

I am thinking about consulting with another RE office for a fresh perspective (I feel like my RE office has written us off)

  • @Wadzfaith, we used assisted hatching along wit icsi with this cycle and the cycle that resulted in our DD.  This was my 6th fresh IVF cycle.  I actually got pregnant 4 out of the 6, 2 resulted in m/c.  I believe the AH & ICSI together was the missing link for us.

  • We also did ICSI and assisted hatching for both of our IVF cycle. I got pregnant with my first fresh IVF cycle that resulted in DS, pregnant again with my first FET that resulted in a M/C and we did a freeze all on the second IVF cycle. We are unexplained.

    I did AH with my successful cycle. Previously, we had transferred a "textbook perfect" embryo without success. With my boys, we transferred two "fair" quality embryos. While we are unexplained, my RE thought our issues may have to do with unknown embryo quality issues. I am no RE, but I strongly believe AH gave our weaker embryos the little assistance they needed to stick around.  ETA: To piggyback on STL's post, the AH didn't change the quality...sorry if that was misleading. The majority of our 22 embryos that never made it to transfer or freeze tended to develop well until the transition from morula to blast. They'd slow down or fall apart at that time. The RE felt that AH might help the embryos, as they wouldn't have to expend as much energy breaking through the shells. And, since they were lesser quality, he felt they needed all the help they could get. Hope that helps to clarify my initial comment.

  • I've done assisted hatching with three of my 6 IVF cycles (IVF #3 which was successful, IVF #5 which wasn't successful, but I had a second line before it went negative again and current IVF #6 results TBD).  Assisted hatching is used before the embryo is transferred to "help" the embryo hatch from its shell.  It doesn't change the quality of the embryo.  But, some women (particularly AMA and DOR) have a thicker shell on their embryos and this helps with that shell.  My first two IVF cycles had beautiful blasts transferred and failed.  A second opinion RE mentioned giving AH a try.  The first cycle I used it on I did a 2-day transfer of our only two embryos along with some immune treatments and it worked.

    ~SAIF/PAIF/Everyone Welcome~ 

    Me= 37 and DH = 41 

    Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)

    IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN

    IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.

    IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132.  Lil is here!

    TTC#2:  Trigger + TI = BFN; Clomid + Trigger + IUI = BFN.

    IVF #4:  BCP + MDLF + Lovenox = 7R, 1F = Transferred 1 6-cell embryo on day 3 = BFN

    IVF #5:  MDLF + Lovenox = 4R, 1F = Transferred 1 10-cell compacting embryo on day 3 = BFN

    IVF #6:  (New RE):  Long Antagonist November 2014 (transferred two 8 cell grade 1 embryos and froze one blast) = BFN

    FET#1:  BFN

  • While we only did IVF with ICSI I have many friends who have done the assisted hatch.  My RE didn't think it was "necessary" for our particular issue, but I was upset when he wouldn't just do it because several friends had done it and had success.  I would absolutely discuss this in-depth with your RE.  There are no guarantees with anything in this game of IF treatments, but perhaps it will be the trick for you.

    Plan: "Quick Start IVF"/antagonist protocol with Follistim, Ganirelix, Ovidrel and progesterone suppositories (yay)

    High AMH (11) and Low BMI= low doses of everything to prevent OHSS

    July 27, 2014: Started Follistim injections for ten days

    July 31, 2014: U/S= 20 follicles, 8.5 lining, E2 is good, Ganirelix for 4 days starting 8/2

    August 4, 2014: 14 follicles on-track, good lining, Trigger 8/6

    August 9, 2014: 9 ICSIed/ 5 embryos; aiming for a 5dt on 8/13

    August 13, 2014: 5DT of 1 4BA almost-hatching blastocyst (Please hang in, Little Bug!)

    August 14, 2014: We have 3 frosties! Hope they're having fun hanging out together. :)

    August 23, 2014: Beta #1: 144 8/23/14: Beta #2: 374 Beta 8/25/14  Beta #3: 6,600 9/3/14

    September 9, 2014: Ultrasound #1 115 heartbeats per minute
