Trouble TTC

What to expect from a urologist...help?

Hi All,
I intro-ed about a month ago and the time is finally here for DH and I to head to the urologist.  Of course the appointment was rescheduled from the original date and they wanted to push it to January (we've been waiting 3 months already) but I think DH pretty much told them I would go on a screaming bitch rampage if they made us wait until then so they "squeezed us in" for Thursday.  Ok back on topic...DH is not the best about asking questions so I'm hoping for some help knowing what to expect.  I know every situation is different but those of you out there who are currently dealing with MIF, what will this dr. do for us?  Will he prescribe me something or DH?  Are there other tests that DH will have to get?  I did an HSG is there a male version (ouch!!)?
So far DH has done 2 SA's plus blood work all came back abnormal.  He currently takes levoxyl too so we already know things are off in the endocrine department.  I know there are a million possible scenarios, just hoping for some idea of what to expect.

  • Our first visit to urologist the doctor reviewed pre existing conditions, discussed all the test results that were done, did an exam, and set up a schedule with when to take vitamins, any prescriptions he should take, and when additional testing should take place. Good luck!
  • I went with DH to his uro appt (ostensibly because English is tough for MH, but really because I knew I needed to hear for myself if I was going to get an accurate representation).

    The doctor was kind of funny to relax DH I think. He had DH take down his pants and did a visual and manual exam right there. He found a varicocele and showed DH how to find and feel it. He made a point that all men live in the same state, the state of denial, Lol. He gave DH scripts for lots of bloodwork and a sonogram. We then had a repeat visit and based on bloodwork results and sonogram (low testosterone and a harmless hydrocele, not a varicocele after all), he prescribed DH hCG shots. A uro wouldn't prescribe anything for you. The uro also looked at the SA, repeat SA, and treatment history from the RE's office.

  • MH went to his appointments alone, so I only got the bare minimum details on what happened. I think prostrate exam and visual inspection were most of it. MH came in with a lot of questions, specifically about lifestyle and diet. Further testing was ordered by the uro. Our situation is pretty extreme, and he ended up having to have a testicular biopsy. After that there wasn't much to be done.

    Not sure how helpful that all is, but I am wishing you both the best of luck for good news and easy treatment!

  • I went with DH and this was how we figured out the start of our Infertility journey. I also will never forget the prostate exam on my poor DH. They will send an order over for a SA and run some lab work on his hormone levels. Our situation showed low sperm count so he referred us to an Andrologist ( male endocrinologist). Best of luck to you both. Any questions please feel free to ask and keep us posted on how it goes.
  • The uro did a quick exam to check for varioceles. It literally took like 60 seconds. The uro also went over my DH's bloodwork results, and prescribed my DH clomid based on low testosterone. He also gave us a list of supplements, and some lifestyle changes like wearing boxers & cutting back on alcohol.

    I was glad that I went, because my DH always hears something different than what the Dr is really saying.

    January 2010 SA results: Count 16 million, Motility 40%, Morphology 2%
    January 2010- Surprise BFP! DS born 10/1/2010 :)

    September 2013 Repeat SA: Count= 1.7 million, Motility= 24%, Morphology= 2%
    November 6th 1st Appointment with RE: diagnosed with severe MFI
    Testing to try to determine a cause & possible treatment for MFI
    CD 3 blood work for me. RE does not want to repeat my HSG/lap at this point,
    but may want to before moving forward with any fertility treatments.
    After seeing the uro, DH is currently taking lots of supplements and clomid to try to boost his count. We will have a repeat SA in February to see if it works.
    Follow up SA numbers are: Count= 4 million, Motility= 40%, Morphology= 1%
    Uro wants us to have another follow up SA 5/9 to see if we see further improvement than we are back to the RE to make a game plan.

    SA 5/9/2014 Count: 12 Million, Motility: 60%, and Morphology 2%. We will be doing iui #1 in late June

    IUI #1 6/28 clomid + ovidrel, post wash count 3 million total sperm= BFN

    IUI # 2 7/21 clomid+ ovidrel. post wash count 900,000 total sperm= BFN

    IVF planned for early November- cancelled due to cyst

    December IVF #1- 22 eggs, 20 mature,16 fertilized

    12/9 Transferred 1 4AA Blast, 6 frosties

    Urologist went over SA history, DH had already gone to an endocrinologist to check bloodwork for Low T. There they took blood work, did a prostate exam and check for bi-lateral varicocele. Our first urologist said by checking he could "confirm" DH had bi-lateral varicocele and recommended surgery. We were hesitant to go straight into surgery and  asked if there is an ultrasound that could be done to confirm. Urologist was evasive so we sought out a 2nd opinion. 2nd opinion conducted same examination at the first urologist and after we had the u/s based on DH SA history placed him on Clomid for 3 months.
