IUGR Diagnosis

Good morning ladies, we had a 26 week growth ultrasound yesterday, Baby S is now measuring 13 days behind which is 5 more than our 18 and 20 week ultrasounds, and I have been diagnosed with Intrauterine Growth Restriction (IUGR).  The plan at this point is weekly ultrasounds paired with biophysical profiles.  If the baby reaches 3 weeks behind in growth and/or there becomes a cord flow restriction we will need to deliver early. I have a circumvallate placenta and a two vessel cord which are the contributors of the IUGR diagnosis.  Does anyone else have IUGR or any experience with IUGR?

  • I am borderline.  At my anatomy scan he was measuring 3 days behind.  At my last appt at 24 weeks I had only gained 1.5 pounds the whole pregnancy and they were a bit worried.  They are going to do a full growth scan next fri and will assess where we are at.  FX for you and your LO....

    ME:46 MH:44 DE IVF 2014
    Met with RE 4/11. 2 IUI's BFN. DE best option. Switched clinics to do "shared" program. Had to retake all tests and a mamm that put me behind and then on a DE waiting list for 12 months. Picked a donor!! (10/13/13) Got matched. Estimated transfer in December. After 2.5 years of patiently waiting I will finally cycle....can hardly believe it. DE cycle got cancelled. One of her tests came back positive.  Waiting for another donor. Donor picked!! (1/18/14)

    DE IVF #1 (4/26) BFN  DE FET #1 (6/4) BFP! Beta 1=339 Beta 2=852 Beta 3=9957 EDD 2/22/15!!

    Thanks!  Mind if I ask.  Have you had an issue gaining weight?  At my anatomy scan my placenta looked good and the cord had no issues either.  My doc is looking out for premature aging of my placenta as my mother suffered from it with 4 of her 5 pregnancies and had 3 stillborns because of it.  I worry constantly that he's too small.

  • I haven't had any experience personally, but a friend of mine had IUGR. She was diagnosed around 25 weeks, was put on bed rest at about 30 weeks and delivered a small, but otherwise perfectly healthy baby boy at 34 weeks.

    It was a roller coaster for sure, but now he's a healthy happy 4 year old!

    So while you may be on track for an early delivery, your OB will know the best course of action for your particular case. OB will help you through everything.

    FX for the best possible outcome and I'm sorry you're dealing with this!

  • I am in the hospital for monitoring one of my twins who we are watching for IUGR.. Bedrest and monitoring twice a day. We have another growth scan on Thursday. They have given me the steroid shots just in case we have to deliver early. So far he is doing really well I all his NST tests.

    Me DX: Hashimotos,Hypothyroid, DOR, MTHFR,  DH: normal

     IUI #1-#4 BFNs and a few cancelled cycles in the mix.
    - poor responder
    ***Suprise BFP on 6/13/13. Natural MC @6wks 3days
    IVF#1 and 2- Cancelled due to no response on max stimms
    FET 5/20- BFP
    1st Beta- 641
    2nd beta- 2166
    Sono- TWINS!!!!
    Two Boys! Born January 2015 @36 weeks.  Healthy and no NICU!  So blessed!

  • I was diagnosed last time at 37 weeks with IUGR. At my glucose testing I did have GD and went on the diet plan. After that I kept losing weight and at each growth ultrasound she was small. At 37 weeks I had a growth ultrasound and they found not only did she have IUGR but I had low amniotic fluid and she was frank breech so we scheduled a c-section that day to get her out so she could grow on the outside. She was born at 5 lb. 4 oz and we took her home from the hospital at 4 lb 11 oz. It was a crazy experience but I have not been told anything about IUGR this time. I should be having a growth ultrasound soon because of my GD diagnosis again this pregnancy. I do not have the early experience like you but I thought I would share. Good luck with your upcoming ultrasound. I will be praying for you.

  • As far as preemies go the mortality drops significantly if you can make it to 28 weeks. Sounds like they are monitoring you very closely and this is what you need! You may need to prep yourself (whatever that means!) for a preemie and a NICU stay. This can be very scary but there are lots of women in your situation with IUGR and the most important thing is that it is recognized and diagnosed and followed.

    It is scary to have the diagnosis but is good that they caught it and you can get what you need. The cornerstone of managing this and deciding on delivery depends on blood flow to and from the baby (Doppler velocimetry) and status of the baby on Biophysical profile (breathing, tone, movement, etc).

    For the ladies above commenting about maternal weight gain, that can be a contributing factor but the usual causes of IUGR are grouped into fetal problems (genetic, infection), maternal problems (preexisting diabetes, hypertension, heart problems, or development of pre-e) and placental problems (single umbilical artery, velamentous umbilical cord insertion, marginal cord insertion, bilobate placenta, circumvallate placenta, and placental hemangioma), with usually multiple factors from one or more categories thought to contribute. The only situation where moms' weight gain or lack thereof is if they had a low prepregnancy weight (BMI <20) and poor weight gain (less than 10 pounds).

    Thanks! This is giving me a bit more hope as all the fetal tests so far have been good.  Now onto gaining 10 lbs in the next 3 months.

  • @chicagojackie‌ thank you for all of that information!!! I am trying to get all of my ducks in a row, visiting the NICU, ordering breast pump, packing bags, mentally preparing, staying positive, and hoping to make it to at least Christmas without the baby being in distress!

  • No advice but t & p that baby can stay put and grow grow grow
  • I delivered my twins early due to IUGR Baby A. His growth was fine at 28w but by 32w had slowed. I started twice weekly NSTs and every other week growth ultrasounds and BPPs. They were more concerned with his general well being and that he WAS still growing than that he was behind. By 36w, his growth had slowed too much so the MFM said I needed to deliver. After he came out, he packed on weight like crazy so it was definitely the right decision for us. Hoping Baby stays on the same growth curve for you.

    Our twin boys arrived at 36w5d due to IUGR and a growth discordance

    FET: Medicated FET moved up to 5/23 due to ovulation
    Transferred a 6BB hatched blastocyst- genetically normal female embryo
    BFP! 5/28- 5dp6dt      
    6/1 Beta #1- 223! 6/3 Beta #2- 567!

  • I was diagnosed with IUGR with my previous pregnancy (DS).  However, it was not until 36 that they caught it.  My belly measured 34 weeks at 36 weeks.  I had an ultrasound a few days later and they discovered DS measured in the 9th percentile.  I was sent directly up to L & D and was induced that night.  There were concerns that DS may need to be transferred to a NICU luckily, this did not happen.  He was born weighing 5 lbs and 14 ounces at just over 37 weeks.  He's a happy healthy 18 month old now! =) Good luck, I hope your story goes as well as mine.

  • I was never formally diagnosed as IUGR but had twice weekly NST's and saw it written in my file when the nurse left the room for something (doc never talked to me about it or made me aware, I have a new doc now).  DD was born at 39 weeks at 6 pounds but has always been in the 4th percentile for weight. She's 2 now and healthy and happy. T&P's that LO grows as long as they can!

  • I am going through that. Baby abdomen measuring too small. I'm 28 weeks. I have another appt in 3 weeks to monitor growth. Every other organ and size is fine except for abdomen. I'm on bed rest and resting well to ensure growth.
