When I'm BFing Isla she sometimes turns red, squirms, flails her arms and legs, claws at her face, pulls off the breast (usually still attached to nipple) and cries...BFing has been hell and my main issues have been dealing with her falling asleep after feeding for 5 minutes (which I'm still dealing with) but now this agitation while nursing is getting worse. I don't think it's an overactive supply because I'm not spraying or leaking when she pulls off and I don't think it's milk intolerance because, while she does have a heat rash looking rash on her upper back/neck, she doesn't have any blood in her poop. I'm thinking either colic or silent reflux? This sound familiar to anyone? Thanks!

Does she gulp, sputter, choke or cough, or gasp while nursing? Does she arch her back and pull away, sometimes unlatching and sometimes not? Does she ever clamp down almost as though she's biting? These are all signs of a forceful letdown.
Maybe try nursing in the side-lying and laid-back positions, and see if she does the same thing then. Both of those positions provide relief to babies who are battling a forceful letdown. If those don't help, perhaps something else is going on and an elimination diet might be another course of action. Good luck!
